Besides, I’m not sure that I will ever be so busy that I close to all payors. I am not going to take any Medicaid after this year & then may close to Medicare too. As for the frequent fliers, why can’t you just say that there is nothing more I’m going to have to offer today that I did not have 2 days ago, assuming of course that the second visit is for basically the same thing (not worsened, etc)? A private office is not an ER where you are forced to see all comers. True, you do have to be careful about putting someone off as they may one day really be sick, but at least a little bit of screening by phone is OK. For example, if someone was in 2 days ago for a cold of 5 days duration & then wants in again 2 days later because it is “no better” I would give them a 15 second phone explanation that a standard uri/cough can last 3 – 4 wks, etc, etc, BUT to call again if not better in a few days or if any worse. If you are not doing ANY screening/triage whatsoever before scheduling then I can definitely see why you would have to close to all new pt’s with only 900 in your panel. I don’t do much medicine by phone, but I also do not want to see someone that clearly just does not need to be seen again so soon as my schedule will eventually clog up & open access will fail. However, I guess your patient example is a good one & if I had a similar patient I might see them more often as well. I do have a few exceptions as well. The pt’s I see quite a lot are those that come back every week or so wanting an extension of their work excuse. A lot of people must really hate their jobs out there! EKG Hi everyone, i was wondering for those of you who have an EKG machine, what diagnoses do insurances reimburse for an EKG? Can you get reimbursed for your hypertensives, diabetics, chest pain? annual physical after a certain age? I am thinking about getting a PC based EKG made by Burdick. Anyone have this? Of all the staff I have hired, this one is the ONLY one that consistently shows up when scheduled, does not make personal phone calls during work to manage her {divorce, bankruptcy, child custody battle, home purchase, or family health crisis} and works the whole time she is here. She has some quirks, no doubt, but if the EKG is the only thing she can’t do in her 10-12 hours per week, I can live with that. I won’t have to get rid of her; eventually she will graduate and go on to full time work some where. Annie Re: EKG Hi Annie, Do you like the EKG from Midmark? Did you look at any other brands ? Here is a thought: maybe she is one of those people who influence electronics in strange ways. One way to evaluate this is to see if this happens to her in other, perhaps similar, situations. If so, she may have an unusual electrical polarity, or something. You may have encountered people like this: computers freeze, electronic doors won't open, etc. If you know anyone who does polarity therapy of other "energy" work, you might as their opinion. I acknowledge that this may sound strange to some, but I feel this is important to bring up since this young lady may have characteristics that may be beneficial in ways we do not understand (yet). Check to see if she has some unsual abilities or perceptive capabilities that have surprised her or those around her in the past. Then again, she could be sabatoging the device, but her overall work ethic examples do not support this. Guest guest Posted January 20, 2006 Report Share Posted January 20, 2006 i completely agree. then she can work in a "wu-wu" office. one of my assistants claims that she can make computers freeze up and even burn down. she once lost all my data temporarily through sloppy mouse clicks. i have found other equipment mysteriously inappropriately set-- "oh, i was just dusting there." amazingly, these things stopped when i told her that they are job-ending behaviors. she is charming, and great with people. she also has great difficulty with authority, and behaves how she wants. happily, she is leaving shortly. I know a woman in LA state who, with touch, can fix computers, electric doors, and other devices that occasionally go on the blink. At first, she did it privately, unsure about how her work colleagues would react. Now, they seek her out and ask her to touch their computers, the doors, etc. More often than not, it works. I cannot explain this, but I have been humbled enough times to know that I do not know everything. My work in medical acupunture, by itself, keeps me on the edge of belief/disbelief every day. It’s only if I leave the room that she has trouble, so I am convinced it is something she does, but I don’t think it is intentional…I told her she needs to recruit one of her class mates and they can EKG each other all night if that’s what it takes, just do it on Friday so I have Saturday and Sunday to fix it again. She does have a good ethic, and as long as I keep her on tasks that use her strengths we are in good shape. Some day she will go on to a full time job somewhere, so I don’t have to fire her.
