Guest guest Posted January 11, 2009 Report Share Posted January 11, 2009 Hi Nic, I did a box of movicol with Ella, as per Cosgrove consultation, but it took 7 sachets for the 1st bowl and I found it too slow, because of the amount of fluid you had to take with it. Also she seemed more stimmy & spacey. Since I wrote and pushed for another consultation, he has now offered to scope Ella or said that he could do a transit study(but Ella can't swallow capsules at the momment). Transit study is swallowing capsules, then 2 xrays. Cosgrove reckons you can tell more from a transit than a plain xray, but obviously the best way is biopsies and endoscopy(but that is invasive for the child & needs general anaesthetic). Plus I wouldn't trust him to scope, as he is not experienced enough with ASD kids. I think the nearest gastro with ASD experience is Italy, he will do upper and lower endoscopy and pill cam-which takes pics along the entire gastro tract. Cosgrove has now prescribed sodium picosulphate, but when I read the packet it has colouring in it, so it is sitting on my shelf unopened for emergencies & I am managing with prune juice. Interestingly though Ella's standard bloods showed a normal thyroid? I would be REALLY interested! According to thyroid sufferers I have hooked up with, these results are very open to misinterpretation. I haven't had time to check fro children, but in adults the TSH should be '1' or lower and the T3 should be in range. It's the T3 that is the active hormone. Practically speaking, the best way to check for thyroid is to take basal body temperature. If it's very low, then that would indicate problems with thyroid. When I was in hospital witn a horrific case of mastitis (8 weeks post delivery) , I had temps of 96 (think that was it) and they really should have picked up thyroid problems back then. (Grrrrrr) Also what was your daughter's ferritin. It is classically low in thyroid problems. Ella has ongoing bowl probs and her TSH was 2.5 (range was 0.54-4.5)and Free T4 14.3(range was 11.0-22.0). Do you have a T3? I understand your not wanting to pique Ella. My daughter would rather lie under a bulldozer than submit to a needle. Do you have a T3? I understand your not wanting to pique Ella. My daughter would rather lie under a bulldozer than submit to a needle. She feels the cold really easily compared to her younger brother who is a porker! Guest guest Posted January 11, 2009 Report Share Posted January 11, 2009 When I first saw a gastro - he prescribed lactulose and picosulphate and told me to administer both of them. Luke had a history of constipation and he didn't bother tell me that I should adminster the lactulose first to soften the stool otherwise the picasulphate will try and contract the bowel and try and push through very hard stools. Luke screamed all night long - it was horrendous - I have never touched the stuff since. Is it a blood test for thryoid? I might try and push for a test? She feels the cold really easily compared to her younger brother who is a porker! Thinking of taking him back to casualty - perhaps see if they will administer an enema. Has anyone ever given their child an enema. Is this advisable? As I said in previous email - he keeps wanting to go to the toilet, but only small amount of diarhoa is trickling out - it really seems as though something is blocking it. What do people think? Does movicol eventually move this? Is it dangerous to leave it if there is an impaction. It was really really effective, it was the only thing that manage to clear him on that occasion. But after that while being on A & E I asked for an enema for my son and they didn't want to give it to him, they told me that apparently it can do them harm (that said on that hospital they didn't want to xray him either....). Enemas are not that difficult to do although with children it usually takes a person to hold them while the liquid is going in as well as one person to administer it. You can do a warm water enema, a soap suds one[something gentle like a small amount of ivory, grated castile] or add an oil product[castor oil/mineral oil] to lubricate things a bit on the inside. If he were mine, I would use about 200ml of warm water with an ounce or so of castor oil mixed in. If you can get him to lay on his left side, you will have the force of gravity working for you to get the solution up into transverse colon. Lubricate the insertion end[a hard plastic tube @ 3 " long here in the states] and then insert it gently into rectum.If you let the liquid flow slowly into him, you will have a better chance of him retaining it. If he begins to cramp, you can stop the flow for a minute or so by bending the flexible tubing[or clamp it off if there's a clamp]and then begin the flow again when cramps subside. If the liquid dose not flow, the tip is probably plugged with poop, si take it out, clean it and reinsert. Once the liquid is in, withdraw the end from the rectum and gently hold his cheeks together to facilitate retaining he liquid a bit - 10-20 minutes would be ideal, but probably not likely. We also have fleets enemas availabel at most pharmacies here - negative is that they are quite irritating and probably not enough liquid to get the job done. You can repeat enemas. It may take a few to clean him out. You might also consider giving some castor oil orally if you can get it down - we had an old recipe that was not hard to take. I'll see if I can locate it. There are also 2 touch points to activate the ileal-cecal valve - right shoulder in the front and right front hip bone. Rub these briskly on bony prominence and with pressure. They will probably be tender. Getting the valve working will facilitate movement in the right direction. The poor guy! Just when I think I've heard it all!! - to sent a wee one home in such a condition seems borderline criminal to me; not to mention abusive as heck!!! All of you who have to deal in such a dysfunctional medical system have my continued admiration and to think 'we' want to implement a like kind here. Anyone remember the movie network? you may all be too young but I get feeling like that broadcaster every so often 'I'm mad as he-- and I'm not going to take it anymore' was his slogan!! Trouble is when we see the doctor Luke is not at his worse - i.e. the really bad pains come and go. Doctors reason for not administering enema: It's a last resort and we haven't given the movicol 3 days to work Sometimes it can deplete the body of water? Usually don't administer it to children under 11 Looking at x-ray seen children more loaded than Luke is My argument was that Luke has been in pain since Wednesday - trying to stress that pain manifests in a different way in Luke than other children and therefore wouldn't it be better to sort it out instead of leaving him in pain! I asked the consultant whether there was an risk involved in leaving an impaction and she said no - only in old people - is this true? Anyway I tried the suppository and it hasn't worked. He has been doing a little more diarrhoa than yesterday and it isn't just trickling out but little bits are shooting out now - so perhaps that means that things will start working soon??? In lots of pain this morning when he woke up - though he did sleep through - though he didn't get to bed till 11:30pm. But after going in bath and having more neurofen - at the moment seems a little more settled. Going to see how he goes today. If he still doesn't shift things - I don't want to wait till this evening till I take him back to A & E, as he would end up having the enema at midnight. So I might take him back by 3ish. There is one for backpackers which descibes faecal impaction as v dangerous but that's not really serious science and didn't go into any detail. The thyroid connection seems to be mainstream science so when this is all sorted you should have no probs getting tests. I posted something on Movicol v other ways of shifting impaction -- Movicol was the most effective. Guest guest Posted January 12, 2009 Report Share Posted January 12, 2009 Hi Nicola We had an enema for our daughter before she underwent a colonoscopy and it worked really well (actually, we were amazed at how present she became when she was off all food and her system completely cleared - definitely dietary issues!) Also, we give Méabh Movicol each day (3/4 of a sachet) and it has really helped with regular bowel movements.
