I was just recently at an AWOONN Conference for Advanced Fetal

Monitoring and this is something that was discussed, maybe they would

have something on their website

Hi Gail,

We have all heard in EFM classes and now know scalp stim during a bradycardia does not help and as it is a vagal stimulus can worsen or at least delay recovery of the bradycardia. Interesting point though... AWHONN Fetal Heart Monitoring Principles & Practices 3rd. ed. on page 164 under Interventions for Bradycardia, "Perform fetal scalp stimulation" is listed. I have access to ACOGs website, nothing about stim during decel came up in a search.

I was at a Risk Management seminar in Peoria this summer, the physician/attorney Dr Jeffery Phalen, giving the lecture showed a strip with "Scalp Stim" written on it during a prolonged decel and simply said "tsk, tsk, tsk".

If you find a good article, please post the link.

Hi Gail et al,

I have heard the same in EFM classes repeatedly. Our OBs stand by doing it based on the AWHONN guidelines, though, as others have mentioned. We had an EFM class last year and the verdict was that scalp stim would remain policy based on said guidelines. We've all seen the impact on the baby from further stress in an already possibly compromised situation (hello, stat C-Section?)--it's hard to believe it's still recommended!

I cannot find any sources (besides countless RNs who see the effects) to state that it is contraindicated!Larissa RN
