Yes, enjoy it while you can. March 31 we had 14 deliveries! Normally, we have anywhere from 0-6. Then on the third, I had another 3. I have no idea what anyone else was doing. LOL

I have seen the journals put out by AWHONN and it looks great....Lots of info available.

MOREOB is Managing Obstetrical Risks Effectively. Here is what I know so far. It hasn't started and the nurse educators/clinical resource nurses are preparing to implement the trial run of it. It is a joint project for nurses and MD's. There are modules on different topics ie PIH, post partum hemmorhage etc. and there are pre and post tests. It is all done via CD-ROM with supports available. It is evidence based theory. That's all I know so far.

What is ANN and NANN? something for neonatal nurses?

That is so strange that several of you have had crazy work days lately.

Things have been weird at our unit too.

I work at a small community hospital with 6 LDRP's and around 900

deliveries per year. I am per diem right now and work 12 hours per week.

I worked this past Wednesday and it was just a weird night. Treatment

rooms busy all night etc. Then right at 5 am (of course) a woman who had

labored beautifully all night with a beautiful strip decided to give us

all MI's. :} Her baby suddenly decel'ed into the 60-70's for a period

of 10 minutes. AAAAKKKK!!! Of course being the small community hospital

that we are we have no 24 hour OR. GRRRRRR! During the middle of it

all women kept coming in to be evaluated, and the twin gest. section was

late for her prep for sx. When all was said and done, decel baby had

apgars of 4 and 9 recovered well, and the section was up in the OR on

time. Whew!

Here's to the thrills of feast or famine OB nursing!

March 31 we had 10 deliveries and we only have 100-120 del a month. Ended up with 17 babies and no room at the inn for our scheduled 2 c/s the next am..they had to stay in recovery till we had discharges. 3 of 6 LDR rooms had postpartum patients.

Guess everyone who delivered had their own special fireworks last July 4th!!! (and that full moon we had last week didn't help)

WOW!

Apparently Friday night was insane at my work place (I was off). This was a case where an on call person would have been a real asset. In a 8 hour period they have 14 deliveries with high risk mom's on mag and a couple sections. With the sections running it left only 3 nurses to monitor 7 highrisk moms/imminent deliveries. It is a wonder sometimes that something doesn't go terribly wrong. THe charge nurse was telling me that by morning she lost her peripheral vision and was having stroke symptoms! Can you imagine!!?!!!

Wow. That is good. My pt last night had fht in the 70's for 5-6 minutes before we stat sectioned her. Apgars were 2 & 8 with the baby in resp acidosis. He had an occult prolapse. Luckily, he was fine once in the nursery after PPV and chest compressions for decreasing HR. I just wonder, what if the HT's stay down and you can't get to OR, what do you do?

I only worked Sunday night. Didn't have time to go back and look, but b/c of the size of our unit and the way these things like this seem to clump together, it is not a usual thing, but has happened. I know only one was greater than 24 wks gestation. We have between 700-800 babies a month and we are a regional referral center for both fetal and maternal high risk pregnanies. Deb

There are actually several things you can do to buy time. It is all easier if the pt. has an epidural. Mainly b/c she can tolerate all the stuff you do easier. Of course pos.on whatever side the baby likes best, O2, increase fluids, D/C pit. If you can really get your hand in there, you may be able to feel the cord that has prolapsed along the side of the head. At least that gives you a dx of what is going on. I have felt this occult prolapse before. Deep trendelenburg can help the head float up and relieve the pressure on the cord. With an epidural, you have to be careful about raising the level of the epidural. I have heard some to the older MD's say they have filled the bladder with sterile water through the Foley and then clamped the Foley, using the full bladder to float the head off of the cord. Then of course, there is the manual way to lift the head off the cord with your hand, I have found with the bed flat or in trendelenburg the cord is less likely to prolapse further. Also you have to be careful not to handle the cord b/c of the possibility of causing a vasospasm which would increase the fetal distress. I have felt a baby gasp in the uterus from a prolapse cord while holding the head off the cord. At this time we were running to the back (while I rode in the bed holding the head up). Very scary. Baby did go to NICU for a few hours, but did well and went home with mom. Luckily we have full staff both in L & D and NICU to do emergency C/S, 24/7, so we can get a baby in trouble out in just a few minutes. Deb

This is what we did. It was frightening yet exhilarating too. I am sure

you all can identify. Exhilarating in the sense that we all worked well

as a team.

Ok, in the space of about 1-2 minutes we...

IVB'd the mom

O2 at 100% via nonrebreather mask

pt on L side in deep Trendelenberg

when that didn't work then

pt on all fours with CNM's hand in vag. holding baby's head back. pt.

still in Trendelenberg

Called the MD and nursing supervisor, and called a stat section.

There was no Pit to dc the pt. had only had 2 doses of Nubain and was at

8cm, -1 station, 100% effaced

soooo close to a beautiful SVD, but the baby had to get ornery with us.

After about 5 min. the CNM pushed Turb. to shut down ctx in hopes of

making the babe happier.

It took all four nurses on duty to get all that done. One of us had to

focus solely on keeping the mom focused and calm.

It's in situations like that that we were wanting our own 24hour OR and

the ability to scrub in ourselves. ahhhhh... L & D in a community hospital

has it's trials.

While waiting up in the OR for the team to come; the FHR went back to the

120's. This was after 10-15 minutes of 60-80's. <gulp> Yeah, and baby

turned out fine. Thank God!

I have felt a baby gasp in the uterus from a prolapse cord while holding

the head off the cord. At this time we were running to the back (while I

rode in the bed holding the head up). Very scary.

Whoa! MI city! That is so awesome that you have the resources to get

the baby out fast! How much less stress would my co-workers and I

experience if we could have that.

I'vr had to do this too, and I never forgot it! To make it worse (and something that got me teased for years) is that our hospital was undergoing renovations so the perinatal unit was on the 2nd floor and L & D was on the 4th. My patient (a then-24 weeker with prolonged ROM and footling breech) prolapsed on the unit during visiting hours. She was already in Trendelenberg. So we rode through the public hallway rolling the bed with me on the bed with a sheet thrown over use--through crowds of people, and in the elevator with several people.

We have several RN's on each shift who scrub, and all of us can open the OR in an emergency.(even if we just open and dump packages on the table and grab the things they may need like IV fluids) One reason we can do that is after each c/s the scrub person pulls all the things needed for a c/s and places the wrapped trays, etc in the OR on the tables and dresses the or bed so all we have to do is open and go.The only time anesthesia is in house during the night is if they have an epidural in place or they just happen to be in the main OR with an emergency. Usually they arrive before the OB does. We have our own OR on the unit, which saves time, we can also call peds to come in from home....most times they can get here within 20 minutes, so the nursery and RT can handle the baby with help from the ER doc if we need him till the pedi comes.

Even though we have in-house anesthesia, maternal fetal medicine, and Neo, we do have an emergency c/s pack that is in the cabinet in each OR in L & D (we have 4 OR's in L & D). On the outside of each pack there are gloves, one of those disposable prep sponges that you just pop the seal and prep, and a disposable knife with a blade on it. That way, the MD can glove, prep, and cut while someone else opens the big pack. It works for us. Oh, and I work in Louisiana. Deb
