Thanks so much for posting this information. As someone who is going through

the nightmare of having to file for benefits, it gave me a lot to think about.

Carols suggestions for applying for and otherwise working to get Social

Security benefits are excellent. I have been involved in acting as a

Rehabilitation Specialist advocating for applicants at all stages of the

process for many years. The best results are generally obtained by showing

that in addition to the physical disabilities, there is significant impact

of depression on one's functioning. This is almost always the case in

chronic pain & it's related disabilities.

I have worked out a set of questionnaire forms for doctors providing

physical and psychiatric care that are made to be exhaustive so that all

facets of your disability are clearly presented. they are best filled out

by the appropriate doctors with you present; and most doctors have been

understanding about this.

I would be happy to scan them and make them available to anyone who is

interested. I would also be willing to help you or your advocate integrate

the information in a manner suitable for Social Security appeals, where

that is needed.

Hi Joanne,

will scan them & send you a copy during the week. Will also place

them in our list file area (may be easier to download that way)

The important thing is to be with your doc (who hopefully is

sympathetic to your situation). There are a lot of technical questions on

the forms. They are designed for Social Security appeals (by me; I have won

40, lost 0 ) It may be necessary for me to summarize them for the

reviewers. As a Rehabilitation Counselor, I am regarded as a Rehabilitation

Specialist by social Security, & that adds a little weight to what I say &

recommend) If you have a good attorney or legal help from Law Servicews

Corporation (part of the Attorney General's office to help people who would

fall between the cracks) they can use the information effectively.

The real key is showing that there is continuing depression that

interferes with all activities, and some indication of thoughts of suicide

(you don't have to act on them). I believe that all of tus have thought

that way at one time or another in distress.

There are a lot of requests. I will do my best to fill them individually

*and* place a file with them in our file area. To make it easier, am

thinking of setting up a page for their download on my website. If I can

compress them enough, they might work on an autoresponder.

The files in my word processor corrupted somehow, so it has become a bigger

deal. I have to scan the material to text. Then I think I will zip it and

send it to the list as an attachment, since so many people have given me

individual requests.

By signing in & checking " Remember Me " , cookies are placed in your temp web

files. If you have done this & there is a cookie problem, contact me in a

few days & I will send the files, or send them as list attachments if that

doesn't bend people out of shape. I have just spent several days converting

the material to usable files (scanning, character recognition, fencing with

a sophisticated OCR program that writes stuff for the web when you don't

want it too & cant change it, etc. All but one form was reproduced well.

aaaall ar ok & useable. I'm just played out right now.

But I sympathize with cookie problems. For a time I couldn't log on to

manage the list.
