I recall seeing a recent study which showed that an uncircumcised man has a four times higher risk of getting HIV from being a top than does a circumcised man. The belief is that it is because of the porous membrane under the foreskin. This has also been put forth as a theory as to why so many heterosexual men become infected in Africa and other countries outside of the United States.

I have also seen data that there is a lot of HIV in the anal canal secretions...so I wonder also if that would not increase chances for an "exlcusive top " to get infected , specially if he has a sore or cut in the penis (or is uncut). But I have also seen a bunch of exclusive tops who bareback for years (many have poz boyfriends or girlfriends) and still remain negative (I used to be a HIV counselor for 4 years and used to give people the results of their HIV test). But I would not take a chance at all if I was negative! This virus brings too many complications in our lives! And it is not fun to tell people you sleep with or date about your status and then get rejected.

Darn tootin', they can! I am a perfect case in point: I got HIV

from topping a hustler the first time I had anal sex! I was 17 and

terribly naive. I didn't even know about lube then, so the encounter

was kinda rough on my penis. I suspect that the lack of lube created

a tear or abrasion for HIV to enter through. Within 10 days of the

encounter I had a severe mono-like illness, which I later learned was

accute HIV infection.

Something else to remember during this discussion is that an undetectable viral load while on HAART doesn't necessarily mean that the semen or rectal mucosa becomes virus-free as well. Yes, a reduction in viral load usually means a drop in the viral load in semen (or vaginal fluid) or in the rectal mucosa, but not always. And just like Brylcreem--a little dab'll do ya. So just think about what level of risk you're willing to accept, and if it's worth it.
