If I understand our Dr. and the most recent study that is coming out correctly 60 & under laying down has a really good chance of full correction so you are in great shape! Guest guest Posted September 29, 2009 Report Share Posted September 29, 2009 Ok someone posted an article that our doctors are publishing and in there they talked about 60 degrees and under and starting before 20 months (I missed that part 1st read sorry) having a great chance of full correction. I don't know if they specified laying down or standing, but i talked to my doctor about why we do laying down xrays instead of standing up ones if the curve is worse standing up. He sad that Min Mehta did all of her measurements laying down and that is what they use in studies so in order to accurately compare studies I think they are all done with laying down measurements. My guess is that this is accurate otherwise how could you compare all of our children when some can not stand and only get laying down xrays. SO I guess I am not positive but that is the impression that I got from combining what I read and what my doctor said. Also all kids are different and some with higher curves and who start later can still achieve great correction and like is often mentioned that not all kids need to get to 0 to avoid surgery or to have successfully benefited from casting. MD [P] Abstract: Background: Serial cast correction by using the Cotrel derotation technique is one of several potential treatments for progressive infantile scoliosis. This study reviews our early experience to identify which, if any, patients are likely to benefit from or fail this technique. Methods: We followed all patients treated at our institutions for progressive infantile scoliosis since 2003 prospectively at 1 institution and retrospectively at the other 2. Data, including etiology, Cobb angles, rib vertebral angle difference, Moe-Nash rotation, and space available for the lung, were recorded over time. Results: Fifty-five patients with progressive infantile scoliosis had more than 1 year of follow-up from the initiation of casting. The diagnosis of progressive scoliosis was made based upon either a progressive Cobb angle or a rib vertebral angle difference of more than 20 degrees at presentation. All but 6 patients responded to cast correction. Nine patients have undergone surgery to date, 6 because of worsening and 3 by parent choice. As shown in the table, initiation of cast correction at a younger age, moderate curve size (<60 degrees), and an idiopathic diagnosis carry a better prognosis than an older age of initiation, curve >60 degrees, and a nonidiopathic diagnosis. The space available for the lung improved from 0.89 to 0.93. No patient experienced worsening of rib deformities. Conclusions: Serial cast correction for infantile scoliosis often results in full correction in infants with idiopathic curves less than 60 degrees if started before 20 months of age. Cast correction for older patients with larger curves or nonidiopathic diagnosis still frequently results in curve improvement along with improvement in chest and body shape. Significance: Derotational cast correction seems to play a role in the treatment of progressive infantile scoliosis with cures in young patients and reductions in curve size with a delay in surgery in older and syndromic patients. I don't know if they specified laying down or standing, but i talked to my doctor about why we do laying down xrays instead of standing up ones if the curve is worse standing up. He sad that Min Mehta did all of her measurements laying down and that is what they use in studies so in order to accurately compare studies I think they are all done with laying down measurements. My guess is that this is accurate otherwise how could you compare all of our children when some can not stand and only get laying down xrays. SO I guess I am not positive but that is the impression that I got from combining what I read and what my doctor said. Also all kids are different and some with higher curves and who start later can still achieve great correction and like is often mentioned that not all kids need to get to 0 to avoid surgery or to have successfully benefited from casting. Data, including etiology, Cobb angles, rib vertebral angle difference, Moe-Nash rotation, and space available for the lung, were recorded over time.Results: Fifty-five patients with progressive infantile scoliosis had more than 1 year of follow-up from the initiation of casting. The diagnosis of progressive scoliosis was made based upon either a progressive Cobb angle or a rib vertebral angle difference of more than 20 degrees at presentation. Update Hello all,We went to ish today for xrays and to see Dr S and get admitted into the hospital for casting tomorrow. We got some good news!! We did standing xrays this time and Dr S compared them to last laying down xray from July. He said (and this confuses me)that upon further review of the laying down xrays from july the curve was only in the 30's (even though in july he had told us it was 47)and the standing xray today was at 50 degrees. I know 50 is bad but i expected 60 or higher so im a little relieved. Also, his RVAD is very minimal!!! Thats great news! So we have a great chance to correct this scoli and hit it dead on! We are at 50 degrees and under age 2 so we have great chances for success! I am very optimistic this really will work! I cant wait to see those numbers tomorrow! Hoping for great correction.He also wants to do a CT scan on Isaiahs lungs to make sure they arent comprimised so he will be having that done tomorrow as well. Under GA of course. Also, we got the doc to approve for us to have a pass to leave the hospital for the night so we all get to stay together in the hotel room tonight and be at the hospital at 5AM tomorrow.
