Friends,

My resolution has been pulled for debate on the floor of the congress. Therefore I will need to speak out in favor of it. Please pray that I have the words to speak out clearly and strongly in favor of women and men learning more about their fertility, so they can appropriately plan for their families. Then, pray the congress will vote in support of this resolution. Thank you!

I am sad to report that the Academy has passed the following resolution regarding supporting policies to provide coverage for family planning drugs. See below:

RESOLVED: The American Academy of Family Physicians (AAFP) support policies and

legislation that would require public and private insurance plans to provide coverage for

family planning drugs and supplies that are FDA approved, including those for sale over the-counter.

We are now discussing the following resolution - unrelated to NFP issues, but perhaps of interest to some of you:

RESOLVED, That the American Academy of Family Physicians (AAFP) support full civil marriage quality for same-gender families to contribute to overall health and longevity, improved family stability and to benefit children of Gay, Lesbian, Bisexual, 1 Transgender (GLBT) families. The reference committee has come out with their recommendations and I believe we have made some positive progress. Specifically, the reference committee has recommended adoption of the following substitute resolution:

RECOMMENDATION: The reference committee recommends that Substitute Resolution No.

25 607 which reads as follows be adopted in lieu of Resolution No. 607:

28 support the development of updated CME programs and patient information resources on

29 evidenced-based reproductive health topics, including natural family planning methods,

32 RESOLVED, That the American Academy of Family Physicians (AAFP) include natural family

33 planning methods’ counseling as a core skill in the Recommended Curriculum Guidelines for

34 Family Medicine Residents: Maternity and Gynecologic Care, AAFP Reprint 261.

While it is not perfect, it is a step in the right direction. However, we still have one more hurdle. The resolution will need to be passed by the AAFP Congress of DElegates this afternoon. Therefore, I ask for your continued prayers in this regard. Specifically, I will pray that it does NOT get withdrawn from the consent calendar for further debate, but rather gets passed as is. If this does get passed it will most likely be referred to the AAFP Commission on Education, where we do have supporters. Then it will be incumbent upon us to translate this resolution into reality by developing and offering CME and working with residency programs to train residents on how to counsel patients about natural family planning

Good work! Be sure of my prayers for the reference committee. It seems odd that one even has to fight this battle, when NFP has been recognized as a legitimate method in both domestic and foreign government appropriations since the 80’s. .

I provided my testimony this afternoon and was grateful there were others who also spoke out in support of it as well. Of course, there were those opposed to it citing the supposed lack of effectiveness and the fact that this should not be required for residents. Therefore, for those of you of faith, please continue your prayers that the reference committee recommend passage of the resolution by the Congress.

I just introduced the resolution to promote education about fertility awareness methods at the opening session of the Congress of Delegates. It required a 2/3rds vote to EVEN be considered by the Congess and it barely passed! Thankfully, though it did, so now I will be able to present testimony at the reference committee. However, this morning's vote reaffirmed that this is not going to be easy; therefore, I appreciate your support, especially this afternoon, between 1 and 4pm when I will be giving testimony. Another is resolves that all community health centers should be required to provide all FDA approved forms of contraception. And, Nurse Practitioner programs usually include NFP training, at least education on the basics.It has been surprising to give a presentation to a group of FPs and IM-Peds Drs who did NOT know anything about it, while the NPs in the audience did. I use the medical literature to back up my claim that it is a legitimate method with scientific basis. I then use a story about a local staff FP who rudely dismissed a couple out of hand when they presented there charts to him. He subsequently had no credibility with this couple and with a large segment of their friends. He basically looked very foolish to them. I state it is important that physicians at least know about the basic fundamentals of the methods, and be aware of the resources in their community for referral of these patients. NFP couples are definitely a minority of the population, are not going away, and they will encounter them. Physicians don’t personally have to agree with the methods, but they must be aware. Dr Duane,I have faith that the Holy Spirit will give you the right words to say. Debate, or more accurately honest discussion, may be just what is needed in order to melt the hearts of those who would otherwise have voted " no " . Once they see that you are not hostile with an agenda, they may be more open to see that it is done out of concern for educating patients & physicians in the latest advances of reproductive medicine. The days of blindly handing out contraceptives because we don't have a true understanding of how to naturally plan our families is over & you have the opportunity to convey this message. What an amazing opportunity! You get to show them that NFP is in no way the archaic rhythm method we are all taught that it is in med school. It truly is the cutting edge understanding of what our bodies are doing & why. Those who are coming from a purely scientific viewpoint should warm up to this message & want to know more. I just can't help but see this as a blessing in disguise. No, it may not be comfortable or easy, but the Lord's way often is not since it often means going against an accepted societal norm. As you stand in front of everyone just try & remember that " His yolk is easy & His burden is light. "

MargueriteTwo rguments that may help1. 30% of couples will face infertility, and sound knowledge of the cycle and fertility focused intercourse the first in solving an infertility problem, and the basis of simple,cost-effective therapies. (Hilgers and Boyle articles will back this up - I have refs if you need them)2. Diversity - for 5-10% of the population - NFP/Fertility awareness for pregnancy prevention is culturally appropriate (Catholics, some ethnic groups). This population needs to be educated beyond the rhythm method and be able to identify the onset and end of the fertile period, and their medical providers need to know enough to help them do this. It discriminates against these people to not provide them with modern, accurate family planning information that is acceptable to

them.(Statistics in textbooks such as Contraceptive Technology indicate that most of the people using NFP use rhythm).Blessings and congratulations for your courage. D3. 3. I think the source of the NFP training for the Nurse Practitioner programs is the Title X Family Planning mandate through the US Dept of Health and Human Services.

Title X Family Planning funded programs are to include NFP as an option. Title X used to fund Family Planning nurse practitioner programs and each program had to have NFP as part of the curriculum. I reviewed these programs for HHS in the early 1990s – to

see if they adequately met the NFP curriculum requirements. The Family Planning nurse practitioner programs were certification based. I also have provided NFP updates for Title X NPs in the past.

Nurse practitioner programs are now at a minimum and the masters level (and most are moving to the doctorate of nursing practice - DNP). Since the masters

requirement was initiated, many of the Family Planning nurse practitioner programs were integrated into Family Nurse Practitioner (FNP) University based masters programs. For example in Milwaukee, Planned Parenthood was the Title X Family Nurse Practitioner

program – they were then re-constituted into the FNP masters/now DNP program at the University of Wisconsin-Milwaukee. I am not sure if these FNP programs all include NFP content – if they are funded by Title X they should be providing that content. I was

asked last year to provide 3 hours of NFP content to the UW-Milwaukee program. Dr. Lee Barron, PhD, RN – who is the director of the DNP/FNP at Saint Louis University could tell us if NFP content is required for FNP programs.

The bottom line is that much of the family planning services for the poor are provided by NPs through the Title X programs. Any health professional, MD or

NP, who provides family planning services through Title X clinics should have a knowledge base for NFP.

I also wish to thank Marquerite (Dr. Duane) for her persistence in working towards the incorporation of NFP/FABM into Family Physician training.

The main argument for teaching FP residents about NFP is that they are going to encounter patients who

use the method once they are in practice. I use the medical literature to back up my claim that it is a legitimate method with scientific basis. I then use a story about a local staff FP who rudely dismissed a couple out of hand when they presented there

charts to him. He subsequently had no credibility with this couple and with a large segment of their friends. He basically looked very foolish to them. I state it is important that physicians at least know about the basic fundamentals of the methods, and

be aware of the resources in their community for referral of these patients. NFP couples are definitely a minority of the population, are not going away, and they will encounter them. Physicians don’t personally have to agree with the methods, but they must

be aware. It might be likened to the teaching of the whole panoply of alternative medicine, so prevalent in our society. Therefore, it should be a part of the education process in residencies.

My daughter who completed a PhD in micro-biology at Michigan State a

couple years ago would teach one session of a class for medical students

each semester. A question she ask was what they would offer a couple who

did not want to take the pill or other unnatural birth control they

offered. " Abstinence " was the only suggestion they came up with. Then

she would tell them to learn about modern natural family planning.

A doctor once told me they had about 1/2 page in their study book on

natural methods. Perhaps the Rx companies are writing their books???
