It is mostly used by retarded physicians immigrated to USA and placed at hospitals' slavery department! I mean AVS without ACTH is not reliable at all...actually if AVS without ACTH indicates adenoma then the patient must have already been exploded by extremely high bp! I am on Spiro=100mg but still take 120 mEq K (=120 * 39 = 4680 mg) and you without bp control are in lower K dose...If you are not on Spiro then get to higher K dose or drink a lot of K-reach beverage/vegetables/fruits until bp is normalized by medication and K supplements...fro now 180 syst bp is too high...you need your kidneys. First, I was on eplerenone all the time, even on three times increased dosage previously to AVS. Second, I sent medical report to Dr. Grim and even he could not figured out from this report, if it was done with ACTH or without it. After that I asked three doctors who saw me in the hospital, including my PCP, and all of them answered they don't know if AVS was with or without ACTH. Now, no wonder, that my AVS showed that both of my adrenals produce the same amount of hormones, and they are practically normal, even if I have 2 cm adenoma on a left side.So, new guy from England, I don't recommend to move to USA or Canada, because we have here the same responses as yours from these two countries. My endo had only used local IRs. When I called the hospital she suggested, I was told that 5 different doctors did the procedure, so figured none had done that many. Thus, I researched hospitals in nearby metro areas. The first IR I consulted said he had done 25 AVS's & only got the right adrenal vein 60% of the time. I chose to have my AVS done by Dr. Georgiades at Hopkins, who had done about 100 AVS's with 96% success at getting the right adrenal vein. I don't think you can do better than Mayo if you can get there. If not, I would research IR depts. in nearby metro areas & set up consults with the most promising. Based on the feedback from this group, in addition to asking questions about how the procedure worked, I asked the doctors that I consulted: 1. How many AVS's they had done? 2. How many times they got the right adrenal vein? 3. Did they use ACTH? In retrospect, I wish I had known to ask if they check K levels before they release a patient. Half way home, I ended up in the ER for 12 hours with a K crash. Another woman in this group had a similar experience. So, I highly recommend getting K checked aftee an AVS. This lowers K going out in your urine by lowering Na intake, DASH is high K and will help replace K. Get low Na V-8 and do 1-2 glasses a day.
