Thanks for a nice explanation. I should hace brought a jug of Low Sodium V8 with me. That will be my plan for next time. Probably didnt help that i just switched to inspra and likely not adjusted to it yet...much less than the Spiro i was taking. ============================================================================45-Male-Caucasian, 5'9"- 242lbs, PA Diagnosed 2007 Suspected Hyperplasia-No tumors on CT - No AVS.Meds: 100mg Spiro, 40meq Potassium, 1800mg Calcium, 500mg Magnesium, 100,000UI Vit D (weekly), 20mg OmeprazoleSide effects: Gynecomastia, stomach inflammationOther Diags: GERD, Hiatal Hernia, Metabolic Syndrome - PreDiabetic, Secondary Hyperparathyroidism caused by Renal calcium leak, Bone Cyct in left

I would do more K and less Na. Again Na goes out in urine. As aldo drives the renal tubule cell to make more ENaCs (holes in the luminal border of the renal tubular cells) which are open to Na and Na rushes into the renal tubule cell. At the blood border of the renal tubular cell a "Na for K process" pumps the Na back into the blood and moves K out of the blood into the renal tubular cell. To replace the blood K pumped into the renal tubular cel (in exchange for the Na pumped into the blood) K moves out of other cells (which are high in K) (brain, muscle, heart). Now your muscles have a lower K and are more irritable and cramp etc. If you breath heavily when singing and playing you blow off CO2 and H20 (H+). This makes the blood less acidic (more alkalotic). To buffer lowered H+ in the blood, H+ moves out of the cell (and K moves in to keep electrical neutrality). These changes may also affect the K and H balance in the muscle and make

it cramp. So my recommendation is to eat even less Na than you are doing now. But then you do not really know if you are at the ideal DASH situation in which you have more K than Na in your urine. So you need to ask your team to check it next time you are there (Urine Na,K and creatinine on a spot urine, timed as noted in previous discussions).Orange juice is not an excellent source of K BTW. Also as it is high in sugar you release more insulin to move sugar into cells. As insulin moves sugar into cells K goes with it. Which may lower blood K. Indeed one emergency way to treat very high K is to give insulin (in the old days). Fairly complex as you can see. Need to see if you can chnage one thing at a time and figure out in you what works the best.

I played a live perfromance last night and had my hand lock up in cramps AGAIN. What am I missing here?I sweat alot when I loaded my gear and when I unloaded/setup. Like drenched. I needed a new shirt.I drank orange juice while I was performing and plenty of waterI was about 90 minutes into my show before the first left

hand lock up. I managed to finish the song with my index finger curled up and a cramp going up my forearm. Try singing and playing with that going on. GRRRRR.I immedately took a break, got another ornage juice, and ate some salt from a table salt shaker.I was able to play again in about 5 minutes, but dealt with the problem for another 30 minutes. Thank goodness my gig was over at that point. What to hell is going on? Am I low on magnesium? Calcium? Potassium? Salt? All of them?I took all my evening suppliments a few hours before my show. Which included mag/calcium/k. I also ate a grilled ham and cheese to get a little extra slat since i knew i was going to sweat like crazy.I even drank extra low Sodium V8 during the dayI dont know what to do anymore. I would have been in a world of hurt if that was a normal

3-4hr gig. When I got home, I checked my BP. It was 104/72 and my pause around 100. I have been on Inspra now for a week. My BP is running about 110/78 on just 50mg per day. Should I up the salt intake? until the pressure starts going up over normal? I stopped the Omeprazole today, as suggested by some on the forum. Maybe it is a magnesium problem? ============================================================================45-Male-Caucasian, 5'9"- 242lbs, PA Diagnosed 2007 Suspected Hyperplasia-No tumors on CT - No AVS.Meds: 100mg Spiro, 40meq Potassium, 1800mg Calcium, 500mg Magnesium, 100,000UI Vit D (weekly), 20mg OmeprazoleSide effects: Gynecomastia, stomach inflammationOther Diags: GERD, Hiatal Hernia, Metabolic

Try switching forms of magnesuim. My naturopath explained Mag comes in mag

sulfate, mag citrate, mag chloride. I saw huge improvement from switching

froms.

low normal and urine is very high in magnesium. But, I cant stand the heartburn

I get when I dont take the PPI. There is no way I could eat the foods on Dash

without it. I tried stopping the PPI the other day and I was suffering terrible

heartburn. I am not messing with that right now. One thing at a time. Too many

changes and I have no idea what is impacting what. I dont think drinking 1/2 a

bottle of pepto bismuth a day is good either. if I dont take the omeprazole, I

wont be eating any fruit or tomato products at all. I can kiss all the LowSod V8

good bye. No grains either. Pretty much be eating just meat to stay alive at

that point of deal with the heartburn. Not just night time heartburn either. It

is there 24x7 and doesnt go away sitting up. I tried sleeping in a recliner, and

will do that if I have reflux problems at night where I basically get puke in my

lungs becuase

> of the hiatal hernia. Burns like hell. I have even picked up lung infections

from it in the past. I have tired all the natural stuff, applecider vinegar and

honey, you name it. I know it takes about 10 days to get past the hellish

heartburn that develops when stoppping the PPI, becuae I have done it in the

past, but I dont have 10 days right now to walk around in hell. I have a busy

week ahead of me at work, new bosses, etc. Cant risk that transition right now.

Maybe I will try it again if a few weeks time.
