Guest guest
Is there significant space between the bone and the plate? Are the screws backing out?TimSent via BlackBerry from T-MobileDate: Thu, 4 Sep 2008 20:13:40 -0400To: <vetrehab >Subject: Fwd: US with TPLO Hi all. We have an unusual case, a TPLO with an infection underlying the plate, in which the osteotomy hasn't healed completely, or at least not enough for our surgeons to feel safe removing the plate. I was thinking of starting US to assist in healing the bone, but am wary given the instrumentation there. Any ideas or experiences out there with similar issues? (Then again, with the infection, US might not be our best bet...) We've been doing LLL for quite a while now. Just hoping to help speed the healing process some. Thanks. Guest guest
HI Amie- We had a similar case with an overweight Doberman a few years back. I'd be wary with the US, as it also stimulates blood flow in the area--could it have the potential for carrying the infection elsewhere? In our case, the surgeon treated w/several rounds of antibiotics, and encouraged us to be very conservative with our treatment, due to the infection. It took a long time, but we did manage to get good results long-term. Good luck! Guest guest
Aime, If you have access to it, PEMF can be very helpful in bone healing...and there's some reports that it can make ABx more effective, too. Guest guest
Hi Amie I would never ultrasound when there is an infection. Guest guest
Forgot to say – you could do iontophoresis with an antibiotic. Guest guest
Hi Amie, If we had a concern about infection, we would not ultrasound, but would treat with antibiotics for one month, then re-radiograph the dog to see if signs of infection are gone. To answer your other question, we have used ultrasound with dogs that have been slow to heal and it has worked very well. We typically ultrasound the lateral aspect of the sugery site for 5-7 minutes weekly for 4 weeks, then perform radiographs at that 4 week visit. All of the dogs we have done this with have healed by that time. We stay away from the medial side of the stifle where the metal implant is. Guest guest
Hi Amie, If you have access, Shock wave will stimulate rapid healing of the Osteotomy site and it used regularly for osteomyelitis. I don’t have access to a hyperbaric chamber so that is what I use! Guest guest
Thank you all for your quick responses. (You all passed my test! No, we're not doing US in this case!) (Just kidding! I'm certain my delirium is due to sleep deprivation as my five month old, Siena, is teething!) Anyways, we saw this pup today and she has a fibrotic/swollen area distally along the plate. She's back on antibiotics again and our surgeon suggested minimizing NSAIDs to ensure healing. On radiographs (which were taken on Tuesday), the distal portion of the plate does have a tiny bit of space between it and the cortex of the bone. We're continuing LLL. Mindy, I'd be very interested in sharing info about hyperbarics with our surgeon and the client, if you don't mind sending it along (including a contact in our area). Have a great weekend.
