Kathie,

1) as for the FHO bone spur left Behind, this is not an unc0ommon issue-this spur is often along the side of the femoral neck (medial) away from the surgical approach, which is craniolateral- In most every cae, this will cause pain, and needs to be removed-it isn't that difficult to do, and will make a huge difference as to the success of any rehab-rehab without tbhis will not imporve the dog, and, the unsmooth area will affect the dog in all pahses of PROM and active ROM-

2) With a meniscal release, i am finding it difficult to point to that as the cause fo the click, especially, 2 yrs postop.- I would also doubt that infection is the issue this far down the road- was the dog radiographed at the time of plate/screw removal, to compare the joint to the inital pre TPLO films- I know that still doesn't answer your question- I would want to know more after you see the dog

{Disarmed} Clicking post TPLO HWR, Bone spur after FHO

Hello Group! I would appreciate any input on two recent referrals.1) I have not seen this dog yet, but this is the information I have sofar. A 7 year old, 69 lb, NM mixed breed that had bilateral TPLOs in2005, with meniscus releases. The dog became lame on the right rear afew months ago, and was initially diagnosed with an iliopsoas strain. The strain was then healed per the DVM, and since the dog still hadlameness, the DVM thought it could be a possible infection at thestifle. The owner wanted the hardware removed from both TPLOs. Ownerstates that there is audible clicking during active and PROM of theright stifle, and that her dog has increased lameness and pain. TheDVM suspects meniscus instability. This DVM did the hardwareremovmal, but not the initial surgery. The vet who performed theTPLOs did meniscus releases. Other than the meniscus being involved, could there be something elseintra-articularly causing the clicking? Have many of you seen thisfollowing HWR? I will know more after I see the dog, and I will besure to assess the spine, pelvis, hips, etc as well. 2) Second question is regarding a 1 yr old, 9.5 lb intact maleManchester Terrier who underwent an FHO 5 weeks ago due to Legg CalvesPerthes Disease. The DVM did not mention a bone spur that was leftafter the surgery, but the owner told me that there is one, and thatthe vet wants to try extensive rehab. I know in the CRI course welearned that if there is painful passive hip extension, there could bea bone spur, and to refer back to the DVM. The dog had discomfortwith all hip movements, but did not yelp until 3 legged standing wasdone after walking over poles, cookies to the hip, manual treatment, etc. So, my question is, can the bone spur cause pain without extension, but with increased weight through the "joint"? Do all bone spursfollowing FHOs need to be removed? I will be calling the vet tomorrow, but would appreciate your thoughts and experience regarding these cases. Wishing all of you a wonderful Holiday!Kathie Touhey, PT-- This message has been scanned for viruses and dangerous content by MailScanner, and is believed to be clean.
