Guest guest Posted November 1, 2005 Report Share Posted November 1, 2005 When we were in public school, I was a bit frustrated with the quick notes that read like this: " good day " or " difficult day " , so I made up a little matrix where the team could rate (high, average, low or no observations) the following list of categories of behavior. No, it's not very scientific, but it was quick and easy for his teachers and provided me with much better quality information than I was getting before. Guest guest Posted November 2, 2005 Report Share Posted November 2, 2005 I'll see if I can dig up what I have used (computer crashed at one point and took down lots of files )and send it off-list IF I find it, but I made up something similar to what Geraldine described with a rating scale from school using MSWord that only needed some codes circled for each behavior/skill area. This was set up for the week with a column for the skill and 5 columns--M-F. I also put a footer at the bottom with about 3 extra lines so they could leave a message, if needed. I also made up something that I sent TO school using a similar format that was also a weekly form that noted how many hours my daughter had slept, ate no/little/good breakfast, used the bathroom before school, was in a good mood/cheerful or not, whether there were new events in family--dad on trip, extra busy weekend, guests at home, etc. That was to give the staff a heads up in case there was anything that they should be aware of, since my daughter was non-vocal at the time. This was all kept in a pocket folder that traveled back and forth each day. The nice thing is that since the forms reflect each other, so when kept together over time I could see whether some things correlated...dad on trip, crying at school more probable...good breakfast, good attention at circle more probable...little breakfast, 'probably need a snack at 10 to avoid a blowout, etc. We could also see patterns and cycles. It helped to make predictions and be proactive. I agree, those " great day " and " tough day " notes are pretty vague, and I didn't want the staff to have to spend a ton of time writing me anecdotal notes. Anyway, it worked pretty well.
