Guest guest Posted February 25, 1999 Report Share Posted February 25, 1999 Up until now we have been very fortunate in our hospital based out patient department that we did not have to fill out the 700 and 701 forms. However as of last week we already received 25 requests. Medicare is asking for the 700 forms, 701 forms, attendance checklist, list of itemized charges and physician signature on our treament plans. Please help. 1.Is it recommended that the therapists do their evaluations on the 700 and 701 forms rather than the eval forms we developed for the outpatients? 2. The list of itemized charges we can easily get from the billing office. 3. Should each patients chart have a calendar in it for the therapists to mark off on the day the patients received treatment? 4. Does the physician actually have to sign off on each and every treatment plan? Can we fax to their office a copy of the 700,701 form and have them sign off on it? Can we have a departmental policy that states the physicians order is their approval of the treatment plan? I will take any and all suggestions.
