The reason why a panniculetomy would aid in weight loss

Hi Everyone!!

Since Jim mentioned Dr. Keshishian suggested a tummy tuck to

kickstart his weight loss and Dr. K had did the same at my consult on

April 2nd, I decided to email the doc to ask him to explain the

theory. Emailed him on Saturday at 8 am and he replied by 9

am...doesn't that man have a life!!!

Here is the theory- Since the body may not be able to distinguish

between the different locations to get the fat from for fuel, It

would be beneficial to surgically remove the fat where is easily

accessible, so that the body will be forced to burn off fat from

unaccessible locations.

Dr. Fobi ( who does the Fobi pouch) presented a paper two years ago

(If I remember correctly), where he recommended that the Fobi pouch

and panniculectomy be done at the same time (based on the same theory

above). In my opinion this is way to much surgery at one setting

(increase operating time, infection blood loss etc.)

There are also surgeons who would recommend panniculectomy before the

WLS. (At my consult, Dr. K mentioned Dr. Anthone prefers this method)

I think the wound management of this is of great difficulty.

In my opinion, waiting for some weight loss before a panni,

significantly will reduce the wound complications. I will also

provide for a better functional status for a patient with a grade 3-5

panni to be able to ambulate, exercise rather than having to wait for

the weight to come off. The reason why TO wait for a long time before

the panni is that it will eliminate the possibility of having some

redundant skin that may need to be removed years later once the wait

loss has stopped. The reason why NOT to wait is that by removing the

panni earlier, will force the body to burn off fat from other

locations, additionally will make it physically easier for patients

to get in to an active exercise program without the hindrance of a

large panni.

Hope this all makes sense.

Hate to tell you this,but yes he does have a life.Believe it

or not WE are his life..lol.. It is so great to have a dr. that

truly loves his work. I think if he could he would live at the

hospital doing surgery 24/7 we are so fortunate to have this

wonderful man for our dr.

The theory does make sence...
