Forms, 700/Eval and others needed

hello,

we are revamping all our forms out here on the island of guam, and trying

to streamline things for the SNF.

i was wondering if anyone could help me out by faxing or snailing me eval's

that have incorporated all the required elements of the HCFA 700 form, we

also need a better flow sheet, D/C summary, screening form, two

week/summary/701.
