This issue has come up for us with Steve. He has gone on field trips with the school and some have been potential stress makers, e.g. Science Quiz Bowl, Math League, etc. Sometimes I put anti-anxiety medication on the form and other times I use the pharmacological name. He is on Effexor right now and so I would write Venlafaxine where you would put the medication rather than put the trade name Effexor which might be more recognizable. For Zoloft you could write Sertraline HCl. You could also write " confidential - information available on a need to know basis only. " I think they are just worried if someone is asthmatic and might need their inhaler or if there is a potential health crisis they would need to avert. How will you dd remember to take her meds? Will someone be there to remind her or ? Many kids are on medications for something or other, it is not so unusual that our kids stick out. The schedule and distraction might allow your dd to have a mini-vacation from OCD and so it is not too likely she needs anyone to know the specifics. We have noticed that those we tell about Steve's OCD seem to forget. Now if he had ADHD that might not work out that way... Sorry I don't know about the legal requirements in MO. I am proud of her, as it could be a bit challenging for a few of her OCD hangups. It is a three day trip. Although we have talked to the principal at her school about her OCD, other teachers and students don't know about it. We received a Health Form that we are suppose to have 3 copies of for her to attend the convention. It asks us to list any medications she will be taking. She is taking zoloft. And although I understand why the convention wants to know the information, I also think her privacy should be protected. She does not want people at the convention to know what medication she is taking (or that she has OCD). Has anyone ever dealt with a health form for an activity for something like this? Do you know if we are legally required to fully complete the form, or does it simply allow them to show no liability if there is a problem with mediation not listed on the form? If it a day trip & no doses will be missed-then it is really up to you-but be aware in an emergency-the first thing asked is meds the child is on, and that would be missing vital info for her safety. OI have 2 special needs kids- one with OCD-I always list all meds for safety reasons-- not worth the risk. I understand about your daughter not wanting to share her OCD with everyone but in my opinion, you should disclose her Zoloft on the medical form because if something were to happen to your daughter the people treating her medically need to know so that they don't give her anything that will interact with it. I always let the school know what my kids are taking and how much so that in the event I am not available they can treat them the best way possible. My daughters understand that doctors and school administrators need to know so that if something comes up, they aren't guessing. The forms should be confidential and if you feel better, you could put on there that her condition is under control and is not a cause for concern. I have girls in my Girl Scout troop who take medication and I have never asked what it is for if I know that it is anxiety related. I respect their privacy and figure that if they wanted me to know, they would tell me more. Congratulations on her being able to attend the conference! But another reason for schools to know and have the form is " if " something happened where medical attention was needed, it'd help the hospital to know what meds are being taken as so many meds have " something " listed as to what to " not " take with them, etc., that sort of thing. So for schools to require the info from all parents is just a safeguard for them, any doctors & the child. They only need to know the medication not the reason; or if reason is on form, I would leave it blank. If she doesn't mind telling what I call " white lies " she can always just say she's taking cold or allergy medication if other kids ask. If she has a little bit of that scrupulosity, tell her to put in her mind she is allergic to OCD! One definition for allergic: " Informal. Having a dislike; averse: allergic to work.
