We definitely think of this booklet as a work in progress. Documentation and the forces that drive it are changing very rapidly. Guest guest Posted December 21, 1998 Report Share Posted December 21, 1998 1. We use functional outcomes 2. We use standardized forms which we created 3. No 4. See attached MedSurgDly Note. At current time staff handwrite because they do daily notes and each page holds 6 notes. (Page is already in Medical chart) so its easier to just do the note while they are up on the floor, rather than come back to the dept. and try to do everyones' notes at the end of the day. In the Rehab area, staff enter info on notes weekly on the computer. We have a central file that outpatient division can access information from d/c summaries prior to the patient's first outpt. visit. All divisions use the same initial and discharge note. The progress note is the one that Rehab uses for weekly reports. We are anticipating starting point of care in April, and will bring on 1 nursing unit at a time.
