Currently my work and job description don't match. I am one of three TFC's at my Hospital and our job has been lumped in with a general accounting position. I would like to offer a rewrite of what I do, but not sure where to start. [(Aguiar, )] My suggestion would be to start w/the role description in the UNOS By-Laws pertaining to TFC's, use that as a foundation, & add whatever additional pieces are necessary to meet the needs of your individual program(s) or center. If the patient chooses to do it at or after transplant some of my superiors are saying that it needs to be in the hospital setting at transplant. Does anyone have other information or could this be clarified. Thank you. Karla A. Keep in mind that it may take up to 2 years for Medicare coverage to become active. Coverage could go retroactive up to 1 year if the patient would have qualified for Medicare at that time. The 2 year waiting period is for people who applied for SSDI for a disability other than ESRD. Guest guest Posted April 17, 2003 Report Share Posted April 17, 2003 I believe that is for patients with Liver disease and the 2 years is for patients who are on disability, this is not for patients with end stage renal disease. The can apply at the time of transplant and the application would be back dated to the first of the month. My issue is does this application need to be done in the hospital, or could the patients apply at a later date or some where else. Karla A. Guest guest Posted April 17, 2003 Report Share Posted April 17, 2003 Our renal network will reject the form if it is signed prior to actual surgery or admission (scheduled living donor) to hospital, and prefers to have it signed the week after discharge. Also, Box 15 question, regarding employment status is misleading for transplant surgery and I have been told it should be answered after discharge but prior to required medical leave or post op recovery period in effect. It is answered based on anticipated employment status after recovery. Other question on line 33, current status of transplant can also be prematurely answered if you have a " sleeping kidney " while patient is still in-house. Our Renal Network requires the 2728 form to be filled out within so many weeks of transplant no matter what, but SS can allow retro application up to 1 year post transplant and are rarely in a hurry to prioritize and complete application. However, I would not recommend this as this holds up reimbursement, increasing AR days. Was told that the notification should be done by either the dialysis center or the transplant department. You may already know this but the 2728 form is just a notification or certification. Even when answering " yes " to the question if patient wants Medicare, another form, Form 43, must be sent as well. Guest guest Posted April 18, 2003 Report Share Posted April 18, 2003 We encourage patients to sign up for Medicare when they begin dialysis, but often they don't, which creates billing problems if they don't sign up well before the 30 mon flip date. That's when the bills start to bounce around and the patients start to receive the bills. What happens if a patient signs up after the first year of dialysis for Medicare A & B? Will they be retro billed at a higher rate for part B from the start date or the last enrollment date?
