I previously consulted with a large rehabilitation and long term care company who utilized Routine Therapy. Routine Therapy was defined as a non-skilled treatment, performed by a Routine Therapy Aide (RTA), under the direct supervision of a Physical Therapist. The RTA was a CNA with additional training by a Physical Therapist, and was an employee of the Rehab Department. Routine Therapy was a function of PT and could be only utilized for those residents " skilled " under Medicare A. (i.e.: receiving OT, SLP or " skilled " by nursing). It was generally recommended after a discharge from PT or when PT was not appropriate (i.e.: resident either too high functioning or too low). Routine Therapy was billed (at a lesser rate) to Medicare A based on the number of units received. My questions are: Is this a common practice in other SNF's? How can Medicare be billed for a " treatment " performed by a non-licensed aide? Is there any written information regarding this topic? (i.e.: regulations, limitations, Medicare Billing, etc.) Any input would be greatly appreciated. Thank you.
