Remember that those forms are billing forms only. The information provided should identify the problems and how they apply to the current onset. The 700 does not replace your initial evaluation. From my contacts, the 700/701 are on the way out the door--FIs are not finding them useful in todays PPS system. When setting up a new system, it would be to your advantage to model forms/documentation within the Standard of PT Practice, Preferred Practice Guidelines, and Guide to PT Documentation, all of which are in the Preferred Practice Reference--PT Magazine November 1997. It's easier to " argue " potential denials if you have documents to support your system.
