Guest guest
Hi J, I think you're lucky to have a doc who'll put you on i.v. vanco. Can't say for sure it will eradicate it all, but I bet you'll feel a whole lot better. Have you tried the betadine nasal irrigation for pseudomonas? My ENT got the suggestion from the Stanford docs and it seems to help quite a bit. Maybe combined with the vanco you'll get more bang for your buck? Just do a search for betadine at the home page and I'm sure the posts with the recipe will pop up. It seemed to work for my pseudomonas, at least temporarily. Haven't been tested lately. pennyJ V <jvitali@...> wrote: This is my first post to the group and need your advise on course ofaction. After two sinus surgeries to remove multiple polyps and after with aninfection for at leat 7 years, I have been diagnosed with the subjectproblem and tests show I've grown CIPRO resistance to these bugs.GENTOMYCIN irrigation only works temporarily. Guest guest
You gotta make sure that your covered nicely with a couple of IV drugs IMO. VAnco/Tobramycin may be a better choice than gentamicin if your doctor does the lab work. YTou also need to do an IV therapy because these problems are bigger and nastier than anyone gives them creditfor..The cirpo is a wonderfull drug for soft tissue but fails miserably with any infection in bony regions. Also rapidly adds genetic mutations to bacteria IMO, that makes the cipro resistant bacteria an absoolute nightmare on your anatomy. Just another note, you need to also cover yourself for coagulase negative staph, 'as importantly ss the other 2', or your therapy may not feel, or work right IMO....Give this whole issue huge respect and don't piddle with it because often people doing things poorly end up worse than what they began with...I would nbever go home with a ten day course of pill drugs and feel I've achieved anything- with this approach your likely to make things a lot worse in the long run as your building towards super infection status and a passport to the autoiimmune world IMO... --- In infections , " J V " <jvitali@...> wrote: > > This is my first post to the group and need your advise on course of > action. > > After two sinus surgeries to remove multiple polyps and after with an > infection for at leat 7 years, I have been diagnosed with the subject > problem and tests show I've grown CIPRO resistance to these bugs. > GENTOMYCIN irrigation only works temporarily. Guest guest
IV vanco for bony regions requires six weeks for succesfull outcomes, alongside some blood thinning agents to make sure the drugs meet the bugs and the bones clear..Irrigatioin like Penny describes is something you should also practise... it neeeds a multi disciplined approach for success. Guest guest
I have been down this path. IV antibiotics never helped me. Had my 5th surgery 3 weeks ago. Dxed with fungal disease by culture. Now being treated with compounded ketoconazol and gentomycin nasal rinse as well as oral Diflucan and Avelox. I also had drug resistant psuedomonas. May have more. The cultures are not all grown out yet. I irrigate twice daily with Breath Ease. After 3 weeks my sinuses are more clear than they have been for 20 years. I think that key for me is that I also have been on a multiple antibiotic protocol for about 2 years for CFS and FMS. Also taking antiviral meds and sups. I think for me it has taken this huge attack on bacterial, viral and fungal infections so once the sinuses are healed I am hoping and praying they stay healed. My immune system was just so worn down I caught all the opportunistic infections associated with CFS. Good luck. If you are in pretty good health otherwise the IV may do it. For me I was just fighting too many battles at once. Have been ill since 1987 and just the past couple of years I am finally finding Drs. who are willing to stick out their necks and work on infections. I am still not well but so much improved. Guest guest
Marie YOU say IV drugs never helped you? I am saying make sure you target your IV drugs, don't do them adhoc, nor do too short a course. And possably most important, be vigilant and keeep up therapies so you don't relapse. MArie we have also discussed some fascinating stuff recently on 'biofilm infections' which make the whole picture more understandable as far as getting rid of these types of infections. The fungal angle is very fickle because antifungals like nystatin work better on pseudonomas than many antibiotics. If you feel strongly you could change the outcome of your disease fungally why not try sporinox or ketaconazole on it's own as I have in the past... to no effect.I'm sure your fungal cultures may highly recommend sporinox a modern marvel antifungal which I don't believe will change the disease process anywhere near what a succesfull bacterial assault will IMO. > > > This is my first post to the group and need your > > advise on course of > > action. > > > > After two sinus surgeries to remove multiple polyps > > and after with an > > infection for at leat 7 years, I have been diagnosed > > with the subject > > problem and tests show I've grown CIPRO resistance > > to these bugs. > > GENTOMYCIN irrigation only works temporarily. Guest guest
D, Thanks for your suggestion. What is coagulase negative staph, a different type of staph aureus? Guest guest
I am negative for fungal infections. Did you have serious side effects with Vanco? How is it administered? I heard it is a bag you carry for the time it takes 3 or 6 weeks. Thanks to all for your advise. Guest guest
I didn't have serious side affects however it wasn't effective either. The doc I had at the time did not grow cultures. A nurse came to my home and inserted a pic line. I had to attach the bag to the pic line twice each day and let it drip into the line and if I remember right I also had to put heparin in the line. The nurse came once a week to check the line. As I recall I had it for 3 weeks. I did have to make one trip to the ER as the site became red and swollen. It is all quite a foggy memory as I was so very sick at the time. Good luck to you. I am glad you don't have a fungal problem. Have you had cultures grown? Guest guest
Whatever you decide on, discuss with your doctor the commonmsense described in the vanco literature by the manufacturer.Never use vanco alone for treating some infections... because you'll fail the therapy. (marie, I think, that had no benefits from this approach) Doctors are rediculously cautious and stupid when it comes to infections, they have been cutting them out instead of trying to eradicate them. The literature explains how to tackle infections and they are guilty of alway's coming up short with lab work and correct treatment approaches IMO. Your basically going to fail before you begin when he states that he'll throw three weeks of vanco at you. The staph I mention can be the worst of the lot and when your doing your treatmenmt and not responding, you may have missed the coagulase negative staph which is easily vanco resistant in my tests. A friend described someone with extensive infection in the head and the ENT surgeon pulled the face off to clear the infection around the skull. The organism growing deep in the quagmire was coagulase negative staph.A common oppurtunistic pathogen that makes many treatments fail when it's ignored. > > > > > > This is my first post to the group and need your advise on course of > > > action. > > > > > > After two sinus surgeries to remove multiple polyps and after with > > an > > > infection for at leat 7 years, I have been diagnosed with the > > subject > > > problem and tests show I've grown CIPRO resistance to these bugs. > > > GENTOMYCIN irrigation only works temporarily. Guest guest
JV By the way, have you developed anything other than a sinus infection ? Are you flat on your back because of this, or are you still functioning nicely? tony > > > > > > This is my first post to the group and need your advise on course of > > > action. Guest guest
I am otherwise OK. I do have Crohn's, and have had surgery to remove 50 cm of cdolon. Are these related perhaps, any evidence? I will ask doc to be careful with Vanco and if she decides to go with the other. Is 6 weeks the preferred time length? Can you carry this around all day or just at night? Guest guest
They have portable i.v. dispensers with automatic pumps so you have very little to do as far as monitoring and can be mobile, etc. Depends on whether your insurance will cover them, because they're a LOT more expensive. However, if you can justify that you need to be mobile, i.e. need to drive, etc., then it's likely they will have to cover it. I had to drive my daughter to school, etc. Vancomycin generally has to be administered very precisely to keep the levels right, so accuracy is important. If you do it with the standard i.v. pole, you'll be hooked up to that pole and limited in your movements for 4 hours at a time. Basicallly, your whole day is pretty much controlled by being tied to the i.v pole, if you don't have one of the portable options. Do a little research for automatic i.v. delivery systems. There are a few different kinds. I had a type called a "ball" with an automatic pump (looked like a baby bottle). Each one is disposable, so that's a lot of pumps, and a lot of expense. This worked great for doxy. I could keep it in my pocket while it slowly diffused the drug without impeding my movements. I'm not sure if vanco can be administered in the balls. It has to do with how much volume you're dealing with, i.e. how much saline needs to be mixed with the drug for proper infusion. But I know there's a portapac of some sort that works for vanco. Just do some research on the types so that when you talk to your doc you can be prepared to explain what you need for your situation. And remember, some portapacs are a pain too, because they can get really heavy, carrying around a gallon of i.v. solution. Believe me, the i.v. gets old, especially after months. I'd also ask for a Grosshong Picc line. They only need to be flushed once a week (although my doc had me do it daily with heparin, just because he thought is was good for me and the line) and it can stay in a long time, unlike most picc lines, which are pretty short lived (good in case you need more treatment). I never had a clog or any problems with mine (other than irritaion from the bandages) and I had mine in in my arm for almost a year. I had numerous nurses tell me they're the best, least complications, etc. You can also go online and look for some kind of device to keep your arm dry while showering. This is a big hassle, and the cheapo ones at the pharmacy don't work very well. I found a plastic newspaper bag held with those stretchy net things they give you worked better than those. But you can buy a nice thing that athletes use when they have injuries that need to be kept dry, if you're willing to pay a little. I couldn't find one in town, but I saw them on-line. The shower thing, and changing the bandages becomes the biggest hassle of having a picc line, besides the actual administration of the drug. penny J V <jvitali@...> wrote: I am otherwise OK. Guest guest
There are a number of articles in PubMed linking Crohn's to bacteria. 11926571 is a good one. "Treatment of severe Crohn's disease using antimycobacterial triple therapy--approaching a cure?" It's not the same bacteria, but I'd bet there's still a link, even if it's just that the IS was depressed by the first infection.- KateOn Aug 3, 2006, at 3:55 PM, J V wrote:I am otherwise OK. Guest guest
Jv Your worried about the way you do an IV drug instaed of worrying about getting your health 100%? You may have crohn's(which means nothing) but an alternative diagnosis is 'staphylococcal enteritis', where your intestines are >>>>>>INFECTED<<<<<<<- I'll just run it past you again, AN INFECTIOUS ENTEROCOLITIS can be caused by staph areus...Wether anyone bothers to diagnose it, is a whole other forum. I would be absolutely excstatic to be trying a major antimicrobial, to see if it impacted my health, as opposed to be worry8ing about inconvenience.
