If you mean SNF benefits yes we do also Home Health, it has been done here for

quite some time.

Hi everyone

Do you check to see if patients have nursing home benefits after transplant?

I have a patient who was covered for the transplant (heart) but not for the

nursing home. In the past I have not been checking this benefit. The patient is

from New York and the company excluded this benefit.

[(Aguiar, )] Sure, for me anyway-I'll let the others speak for

themselves.

I have been in tx since 11/93 when I swapped jobs w/someone. (Talk

about not knowing what I was getting myself into!!) At that time I was

working @ Massachusetts General Hospital & continued in there until April

2000 when I came to Beth Israel. While @ MGH I worked w/all the solid organ

(kidney, KP, lung, liver, & heart), as well as BMT. Owing to the lack of a

Tx Administrator, I dealt w/many issues not normally handled by the TFC,

including financial analysis of the programs & contracting issues. As I

said previously, I left MGH in April 2000 to take up a new position at Beth

Israel.

My UNOS activities can be traced back to 1996 when I joined what was

then the Pt Access Subcommittee under Tx Administrators. (TAC) The

Subcommittee's purpose was to identify non-clinical issues which might

preclude a pt's access to tx services. Among the issues identified by the

Subcommittee was the variability of TFC's in tx ctrs nationwide. We felt

this variability stemmed from the very vague role description contained w/in

the UNOS by-laws @ the time pertaining to TFC's. Together with

, at that time a TFC w/Univ of Chicago Hospitals (now a nursing

student), we undertook a survey of all 272 UNOS member tx ctrs & their

TFC's, including staffing levels, training, & practice patterns. Results of

this survey were presented at the UNOS Tx Mgmt Forum in San in 1998.

Using this data as a foundation, we were able to facilitate an extensive

revision of the UNOS TFC by-law, which became effective 6/99. BTW-the

Subcommittee has recently evolved into a full-fledged Task Force, which I

continue to participate in. I have sought the input of members of this

group for a project I'm presently working on involving the exchange of

information between the TFC & case mgrs.

At the last Tx Mgmt Forum in Savannah I was asked, along w/a,

Chaney, Amy Kaye, & Kent to facilitate a " Sunrise Session

for TFC's " . More than 40 TFC's stumbled out of bed to join in for some very

lively discussions. From that early morning session came this mailing list

(which was originally based in my e-mail program until I moved it to eGroups

in June), which itself is the foundation of TFCA. Due in large part to the

turnout in Savannah & the activity of this group, TAC decided to schedule a

TFC-track for the SLC PCW, which I have been organizing, along w/a break-out

session in the Main Forum, which I am also organizing.

Additionally, I have authored or co-authored the following papers:

our txp financial coordinators do not check nursing home benefits as a rule. we

have a utilization review dept and a socia work dept that get involved in the

after hospital care stuff. on occassion we are asked to do a medicaid app for

those pts requiring after hospital care who do not have benefits for it... rusty
