Input needed for BCBS appeal

Greetings,

I'm finally ready to schedule fusion for BI at TCI, but I would like

to start the appeal for coverage first.(BCBS PPO, Regence,Washington).

How much info do I give them?

How many details?

Do I include the copy of my missed DX from local Chiari expert?

I do have a copy of the on-site CCI insurance file and will also query

TCI for help on this, but I feel help from those of you who have been

through this would be invaluable.
