Sharpe & Wessely (the insurance company 'puppets', IMOHO) are still in the forefront of continuing to trivialize this DD as psychologically based...yet several notables, inclusive of Klimas (Fla.), Kamoaroff (Harvard), Enlander (NYC), are mentioned in the retort. There are many good medical citations in support of the document therein. 2) For those who like to do searches on Internet for medical issues of concern, the following resource is now available. It also has a link to PubMed which has a lot of pertinent references.
