I'm treating a patient for a chronic, unresolved UTI, dating back to an

acute GI infection about 20 years ago. At the first appointment he

mentioned that he got occasional little itchy blisters on his wrists and

around the bases of his thumbs.

The urinary tract problems are resolving nicely, but the skin problem has

gone mad. The blisters turn into dry, red, itchy patches with deep cracks,

and his hands have swollen alarmingly, especially his fingers. The swelling

is in the soft tissue between his palms and the first finger joint, but is

also starting now in the creases of his elbows. The blisters look like

pompholyx, but are in the wrong places.

I've changed his medication to deal with the skin problem, but it is still

getting worse at an alarming rate. I must be doing something wrong here -

all advice gratefully received.
