Guest guest
Good Morning Everyone Since our web site is up we have been getting many Emails asking about transplants for foreigners from several different countries. Does anyone know the UNOS policy on this? Also do your centers collect $ up front prior to listing for the full amount? Thanks >>> LAguiar@... 02/09/01 12:06PM >>> hi suellen... welcome to our shared nightmare. [(Aguiar, )] Great way of putting it, Rusty! :-) Suellen, I can tell you that @ my previous facility (I've been here for just under a year), we had decided when we started the living donor lung & pedi liver programs (1995) to mimic the existing Medicare policies for kidney tx. We set up both a living lung & living liver donor cost center @ the hospital, accrued the donor charges there, & then @ the time the recipient was d/c from the tx admit, the final total for the donor was added to the recipient's inpt bill as " organ acquisition " . (Three guesses who got to do all this, btw!) You also need to keep in mind that the OPO up here handles CAD charges differently that most other OPO's in the country-this was another factor in our decision to set things up the way we did. We continued to assume the costs of the donor's post-op visits under the donor cost center. Thankfully, we had never run into a situation where the donor needed to be re-admitted (though curiously, it did happen on the renal side!). At the time I left, the ctr was gearing up to get started w/the adult-adult program & I believe the plan had been to continue in the manner I described w/this population. However, they never did back-fill my position & I've heard the first adult-adult LRD liver they did after I left was denied. (Coincidence?) I do know of another ctr who seeks approval for each individual piece of the donor testing from the recipient's ins & bills each approved test to the recipient's ins-I just am not sure of their payor mix or their success rate. Bottom line is that, as Rusty said, this is our collective nightmare & everyone has a different method of dealing with it depending on your individual center culture. This is one of the reasons we'll be looking for people @ the TFCA mtg in SLC willing to work on a subcommittee to petition HCFA to start covering this as well as issuing guidelines for the donor reimbursement & billing pieces. Guest guest
If it's for pre-tx evaluation or the transplant admission we pay it and then put it thru Kidney Acquisition (on the cost report). If it's for the professional fees for the admit, we have the provider bill the recipient's Medicare directly (it's covered thru Part a TransLife Hi Group: I have a question for you all. How do you handle the balance on a donor bill when Medicare is the secondary payor for the recipient? Guest guest
Good morning, I attended only the first day of the UNOS Management Forum. That Monday a question was posed to CMS concerning illegal immigrants and funding transplant. I believe we were told this issue would be discussed later in the week. Was this topic reviewed? I now have 3 patients who have private insurance but who are in this country illegally. Have any of you experienced this situation and how did you resolve it? My concern is that they will not qualify for Medicare and if they loose their private insurance post transplant......... not to mention donor issues. Should I visit my local INS office for further info? Thanks to all--Happy Tuesday! Guest guest
Good afternoon, We have not had may illegal immigrants, but have had equal problems with legal and students. The person must be entitled to medicare, this is a problem most have not worked enough quarters for this. Students who are in the states on a student visa did not qualify for medicare and no state programs because they were not legal residents of the state of PA. We had one pt who followed his Nephrologist from one of the Carribean islands, he qualified for state aid for dialysis, not for medicare and medicaid would not cover the transplant because he came here for medical treatment. We have the patient sign paperwork specifically geared to there particular problem that they are responsible for the medications post transplant. Although even if they have money set a side for a specific period of time, they seem to eventually run into problems. These are pts here legally, when not a US citizen they are limited to help they can receive. At least this is what we found at this facility. I am meeting on Monday with the Director of Surgery to discuss a new Transplant Administrator position that we are recruiting for. But what I am really looking for is an organizational chart of your transplant department. If not, can you just give me a list of the staff that your transplant department has? This would be very helpful for this meeting. Guest guest
Check with your medicare intermediary, is your facility trying to become a medicare certified facility? Rita McCann Administrative Financial Coordinator Lankenau Hospital Good Morning All.... We have a patient in our facility who is Medicare primary and Keystone Mercy secondary . She is in desperate need of a transplant like now. However we are not a Medicare certified facility. She loves the doctors here and refuses to go to Medicare certified one. She was hoping we would be certified before it is to late. Which it is now. We have gotten the hospital to absorb any fees associated with the transplant. Now.. what I need to know is will Medicare pay for her immunosuppressant after the surgery?. I am getting conflicting information on the subject. If anyone has information? I would greatly appreciate it. Barbara To unsubscribe from this group, send an email to: TxFinancialCoordinators-unsubscribeegroups Your use of Yahoo! Groups is subject to the Yahoo! Guest guest
there a lot of commercials where i have to change the station or grab the remote and 'MUTE'......songs like on the el pollo loco commercial, also, one where a HUGE ZEBRA with a HUGE TONGUE is licking a glass door 'GROSS'.................many more that i can't think of right now but i also go crazy if i even see someone about to eat within earshot of me. i left the waiting room of my dr's office when a family decided it was " lunchtime " in the waiting room. this has happened to me many times. if i see someone taking out a piece of gum, i have to leave, knowing what's coming. it's really gotten worse in the last couple of years. i cannot visit my best friends house anymore because her daughter's eating habits are scraping the silverware on the plate, chewing with a huge mouthful of food even to the point where it is spilling out and talking while eating. her mom noticed that i " cringe " when i even see her when i visit because at some point, i know i am going have to either listen or " pretty obviously " AVOID being near her when we eat. i tried explaining and she knows i have this but i think it may have cost our friendship. instead of trying to teach this 21 yr. old manners, she told her i have a " mental problem " .........O.K. THIS SUCKS! Guest guest
YES, FUCK HER AND EVERYONE WHO LOOKS LIKE HER. It is getting worse for me as well. Today there is an " environmental?? " truck outside making a very loud noise and this will last all day. It is there to suck out something from the restaurants across the street. I have complained to the EPA and they actually drove out here one day!!!! (Manhattan) but that day the truck was not there! Oh well. I was surprised that they came to check out the noise but that was the end of that. I never know when the truck will be here. I cannot imagine life for people living on this block. Heck I am here til' 5 and can barely stand it. I am the one who was reprimanded on my 3 year anniversary here at the magazine for complaining about noise. I have no more chances. One more complaint and I am out of here. There is something wrong here. I am a hard worker and it is distracting when others are on the phone talking to friends. I GET IN TROUBLE but they don't and they continue not to work. I guess I have to invest in those $300 headphones now and look weird. Told my co-worker I will give her a bunch of rocks to throw at me if she needs my attention while I am wearing them!! pia sorry about the language!! > >Reply-To: Soundsensitivity >To: Soundsensitivity >Subject: Re: >Date: Mon, 1 May 2006 02:06:18 -0700 (PDT) > >By telling her daughter that you have a 'mental >problem' she simply means that you aren't an idiot >like her or her daughter. Guest guest
Earphones work good to me, and when someone needs my attention they yell at me or point at me or something like that. I put my music off and listen to them, talk back and putt the music back on. it doesn't have to be very loud, the music, bust just hard enough to not hear sounds from outside and focus on the music. I, as only one at my school, have oermission to wear the earphones while making exams, at lessons.. everywhere. Almost no one knows why I wear them, but they don't look weird at me anymore. In restaurants i wear it too, poeple are getting used to it luckely. And if co-workers ask why you wear them, you have a good reason. Your boss must understand it. Guest guest
Jeroen. Due to the fact that you have no children makes it possible for you to freely use these devices to block the outside world. I would never get away with that since my 4 year old is with me nearly everywhere I go. When I worked she even came with me. I had a nice boss! I suppose it would make me a bit neglectful of her If I kept those on all day. At the present she doesnt bother me though, (unless she is whining BUt I believe everyone, not just sss sufferers dislike whining)so I never have to shut her noises out. It would be very nice to have that when my husband or son are home though! Also, luckily for you, People never look funny at a young person listening to music all day. My teenage son spends the majority of his waking hours with his ipod attached to his head. I say use them while you're young and have your freedom to do so and hopefully once you have children they don't bother you. Your lucky your boss allows this. WHat kind of work do you do? > > > > > > > > > > > > > > I found you guys through a Live Journal site I > > > > discovered today. > > > > > >So, > > > > > > > I'll just repost my intro post from there > > > > > > > > > > > > > > I found the LJ site because I was searching > > > > online to see if > > > > anyone > > > > > > > else has the sort of reaction I have to food > > > > noises in > > > > commercials > > > > > >and > > > > > > > television shows.
