When gram NEGATIVE bacteria die en mass, they give of chemicals that initiate a systemic cascade throught out the immune stsem that results in fever and sever flu-like symptoms. I'm not saying there can't be localized symptoms from die off- and there can proabably even be adverse symptoms from a bazillion of them REPLICATING.. but they don't fir the definition of classic herx. It's very rare to have a Herx from the die-off of gram POSITIVE bacteria (although there are instances of sepsis with some gram positive species) IMO- if you have a heavy load of gram positive bugs - and you kill them off with high dose abx, you feel great because they're dieing, and becuase you are NOT herxing. This is the case when one takes abx for bronchitis, or pneumonia, bladder, prostate or urinary tract or infection even an infection that's gotten into the blood... People feel better as the days go on and the bugs are dieing. That's my opinion anyway.
