Arlene,

I don't have a copy of the reg you are referring to, but I do believe

that you are correct. As long as the patient has Medicare primary at

the time of tx, they should cover the immunos. If there is another

insurance primary, they need to be billed first. If there is no Rx

coverage (or the cap has been met), then Medicare can be billed with a

denial from the insurance, or a letter from the insurance company

stating that there is no Rx coverage thru the primary insurer. We've

had to do that in the past and upon review, Medicare will cover the

claim.

tions unless Medicare coverage was in effect at the time of transplant and they

paid for the transplant. A copy of the Social Security Act regulation

regardingthis matter was faxed to me in March 2000. Does anyone know anything

This is my argument exactly with the patients, but most of the time they

cannot afford the premium, so that is why I tell them they could than wait

until a transplant occurs. I have Medicare applications waiting in their

file and submit it to our clinic on the day they are transplanted.

Not only that, if they don't apply for Mcare at the time they are eligible,

they are penalized for not enrolling right away. Also, most insurance do

not cover donor expenses whether live or cadaver.

Thanks a, I appreciate the information. Even if the patient had Medicare

as secondary at the time of transplant, they would probably pay as such up to

their maximum allowable charges if the primary insurer's reimbursement did not

come up to that amount. So, if I understand you correctly, in order for

Medicare to pay for immunosuppression medications, they would have to be either

the primary or secondary carrier at the time of transplant. If it's anything

We always check with the recepients insurance to see if the donation expenses

are covered and get this in writing. I would like to have all patients also have

Medicare but many are reluctant to pay the preminums.

I disagree with you on the Medicare issue. I firmly beleive that the

recipient should opt for Medicare (both A & B) prior to tx. If they

have a living donor which is not covered by the recipients primary

insurance, they MUST have Medicare B in order to bill thru kidney

acquisition for the donor eval. We had discussed this very same issue

several months ago, and the general concesus was that it's important

for the recipient to have Medicare during the eval process and not to

wait until after the tx. The small amount they have to pay for the

Part B premuims is well worth it. Medicare part B also covers the

immunosuppressives if their primary insurance has limited Rx coverage

and it also covers the living donors post tx followups both

out-patient charges as well as professional fees (as we've been

discussing).

I tell my patients the importance of having Medicare, especially if

they have any potential living donors. Providers have been " burned " a

few times when the recipient did not have Medicare B, then tx occured

with a living donor, and their primary insurance denied donor charges.

It can turn out to be a mess.

I have another question. I'm new at this and no one here seems to know. Can

Medicare billed for donor services provided immediately post op by VNA's if

Medicare is applied for at the time of transplant and no card or number are

available yet?

I agree with a on this one. The cost for Part B is about $46 per month, and

a small price to pay for the benefits of coverage for diagnostic studies,

doctor's visits, etc.

Help . I am confused. I think we must be doing this all wrong. We pay for all

donor evaluations from the kidney acquistion cost center, for both patients who

are Medicare primary, which we have very few of since we transplant most

patients before they even start dialysis, and those that have commercial

insurance primary and don't yet qualify for Medicare. Should we be billing the

recipients insurance of the workup of the donor and use the kidney acquisitiion

for Medicare patients only? These procedures were in place when I got here.

Visiting Nurses or Home health Agencies. I'm sorry. I forget all parts of the

country have different terminology for these services.

Not only that, if they don't apply for Mcare at the time they are eligible,

they are penalized for not enrolling right away. Also, most insurance do

not cover donor expenses whether live or cadaver.
