I made my off of s ABC's book sample and by looking at a few others off the internet (Medela has one) and my own spread sheet. It was a bit of a hassle. I don't have it on my computer because some of it was made by me and some was old fashion cut and paste... I know you can pay for someone to make it for you Pat . http://patlc.com/LVR/ Here is the problem. These codes are the same as in a Dr. office. So the codes can only be used so many times.
