Guest guest
Update: Baby had a 2nd laser frenectomy on Friday. Apparently it grew back. I did recommend a re evaluation 3 weeks ago and she just went this last friday. Mom reports baby seems to be chomping at the breast WORSE than before. OH MY GOODNESS. She is coming to see me tomorrow. This baby is now 10 weeks old. How can I help him learn to use his tongue? She is doing suck training with him as I showed her my first visit over a month ago. Baby has had many chiro adjustments. I really want to help her but am at a loss. Liz Pevytoe RN, IBCLC > > I have a client that I need help with. Of course I have her permission to share/ask for help. > > Hx of pain/injury from day 1. I saw her for the first time at 6 weeks after baby had been lasered for a tongue tie 10 days prior but was still having pain. She has had mastitis and plugged ducts from the start as well. She was also diagnosed with thrush a few days prior to our appointment. Her supply was almost non existent at our visit because she had been pumping and bottle feeding due to extreme nipple injury and pain. > Poor momma was completely depleted at our visit. Baby does not use his new tongue properly and he chomps very aggressively while nursing. NO jaw thrust, up and down hard chomping. Mom had thrush and deep nipple ridges that were inflamed and " screamed " pain. > > Baby has had cranio sacral therapy and chiropractic adjustments and continues to chomp. I also have instructed her on suck training for after frenulum revision. > Mom developed vasospasms, and were resolved until we attempted relatch this week and now they have returned. She does not have vasospasms when pumping though. > Good news is supply has rebounded (with use of Motherlove) and nipples are partially healed but everytime we attempt to relatch baby re-injures nipple with the chomping, despite effective " looking " latch. > Baby is using a wide mouth bottle, with paced bottle feedings and yes he chomps on a bottle too. (We did finger feeding for a week too and there was no improvement with chomping then as well.) > This is not my first rodeo so to speak. I have been a LC since 2003 and I have tried every trick possible (that has worked in the past) with this child to no avail. > > Please help me help her, she has been through so much and I refuse to give up. > > Liz Pevytoe RN, IBCLC > > P.S. I am so glad to have you guys to discuss difficult cases with. It is such a gift. >
Guest guest
I don't know Liz, but I hope you find a good answer for her because I could use a similar answer for someone else. Lea
Guest guest
This baby may be uncomfortable. Dr Larry Kotlow recommends not only oral massage and stretching post surgery but also pain medication if needed. http://kiddsteeth.com/nursingbookaugfc2011.pdf Ellen Simpson, IBCLC Tampa FL From: [mailto: ] On Behalf Of askthelactationconsultantSent: Sunday, August 12, 2012 9:39 PMTo: Subject: Re: Chomping at the breast (update) Update: Baby had a 2nd laser frenectomy on Friday.
