I saw a mom last week for a 10 day old baby with latch issues. Mom complained of sore red nipples. On inspection nipples were red with L had eraser size piece of tissue missing on lateral midline of nipple. Ouch! Mom states nipples were bleeding and cracked but were healing. Both breasts engorged. Hx of implants. Oral inspection baby clamps/ strong suction. Tongue movement was good with tongue placement past lower gum line at latch. Latch was very shallow. Position was good. Lips slight flare. Latch corrected with more wide open latch. Mother stated it was a little better, a little less painful. Nipple shield used with 2nd latch mother stated it was much better. Baby clicks at beginning of feed. Mom encouraged to pump/ hand express before feed/ warm compress. Cool compress after feed for engorgement. Mom taught reverse pressure softening. Mom wants to use shield until healed. Mom instructed to dry nipples after feed, then express and leave milk to dry. Infant transferred 2.0 oz in 25 min feed with shield. Baby referred to CST. Mom just called and said she cant do this anymore it still hurts. States latch is much better and nipples are almost healed but it hurts. Birth was a section for FTP with suction for meconium. Infant given O2. So what an I missing? Could it be a hidden TT? I am hoping the CST will bring out one is so. Mom also took baby for adjustment and said baby was " tight " .
