Dearest Ladies:

Here are the instuctions for sending explanted breast implants and capsules to Dr. Blais. I'm sorry that this did not come out better, but it was for someone else and I had cut our her information. It is always better for each lady to send a fax to Dr. Blais and then he will send you this information. It is always better to have the information directly from him.
