Platinum Analytical Urine Sample Collection Protocol

1. A urine sample of the first morning urination

should be placed in a sterile collection vial.

2. A chain of custody form should be completed by the

patient. This form may be downloaded from the Internet

from our webpage Expertox.com

3. The urine sample vial, along with a similar empty

vial (for blank correction), and the completed chain

of custody form may be shipped in a small cardboard

box containing blue ice for temperature control during

overnight shipment.

4. Please exercise caution in ensuring that the

specimen vial is securely sealed, in order that

accidental leaks do not occur during shipping, and

avoid the testing.

5. Place a $150.00 Cashier's check/Money order in the

shipping box.

6. The results of your testing will be mailed to you

in the address you have included in the chain of

custody form unless you instruct us otherwise.

ExperToxT Laboratory Services Urine Collection

Protocol Urine Specimens can be collected for drug,

nutritional, chemical exposure and forensic litigation

testing:

Follow is the collection protocol for providing an

acceptable specimens for testing:

1. Get a clean, sterile (if possible) plastic

container with tight fitting lid.

2. Catch a mid stream clean specimen approximately 45

ml or 1-2 ounces.

3. Cap the container tightly.

4. Place container in a second sealable biohazard bag.

5. Place the sealed specimen along with the

Requisition for Testing in a small box or bag and send

to ExperTox laboratory in Airborne Express or other

overnight courier container.

6. Call ExperTox Laboratory if you have any questions

at 281-476-4600.
