You can file this report on your own, without a doctors signature. Please, please do so! - Rogene ------------------ From Carolyn Wolf This should be an easy way for people to either do an Adverse Event Report -- online (print a copy before you hit the send button)--or the form can be printed, and blanks filled in... Most everybody knows the type of implant -- approximate date of implantation, date of explantation -- when Rheumatoid Arthritis was diagnosed -- or MS -- or Sclerderma...fill out a form for each diagnosis -- EACH event is an adverse event..If you choose to mail, you just fold the second page over the first so that the mailing address is visible, -- you don't even have to buy a stamp!!
