Pump flanges for hypoplastic breasts?

I saw a mom tonight with classic hypoplasia. She has an Ameda Purely Yours

(which I have less experience with than Medela) and was using the 25mm pump

flanges. The flanges are not a good fit -- lots of areola getting sucked in to

the tunnel, rest of the flange is not forming well to the breast. Suggestions on

best flanges for moms with tubular shaped breasts?

***feeling sad for this mommy... seriously TT, 7 day old, BW 8-1, yesterday at

pedi 6-14, pumping 10mL total and passionately wanting to exclusively BF.
