RE: skin eruption

any thought appreciated.

woman aged 41 arrived with skin eruption of only 5 days standing and

rapid progression. all and only distal phalanges showed marked fissuring

following small papules with thin exudate and crusting. intensely itchy

intermittently, much worse at night. also 'quite sore'. all nails lifting

off, almost as far as the bottom of the nail. also discrete small itchy

lesions like insect bites across chest and shoulders. complains of itching

in both axillae and she has a bilateral patch of raised shiny thickened

erythematous skin similar to a patch of contact dermatitis under her jeans

stud. she has a known nickel sensitivity which has not previously manifested

systemically. recent contact with a child with chickenpox. no other history

of skin disease. i suspect there are at least 2 distinct dermatological

problems here.

I must confess I was rather distracted by the rest of her - slight

swelling of throat, exopthalmos, tremor, hairless axillae, pulse 104,

palpitations, SOB, scanty infrequent periods, overheating, hungry,

tired....she had no idea!

I treated the skin aggressively with anti infectives and lymphatics and

alteratives adn locally with comfrey cream mixed with commiphora and

hydrastis powder; essential oils naiouli, helichrysum, palmarosa. - assuming

it was impetigo or similar (the second woman with it on her hands this

week). I was rather alarmed by the nails and can find no reference to such

dramatic nail involvement in infection of any sort. or to cutaneous

manifestation of thyroid disease.

Hi nne,

I realise that this px is quite bad off but. I am also sensitive to

nickel and is very sensitive to different washingpowders. Recently I

decided to try a new fabricsoftener out, and voila the following day

my skin had errupted on arms and shoulders, very itchy, small papules

with a watery exudate - painful when itching gets a bit crusty after a

while. They will not go away but has improved dramatically when redid

the washing with old detergent. Mind you in your case there is

probably more to it....

Thanks Stina. in fact she hasn't changed anything at all. i sent her to the

GP for thyroid evaluation but I think we know the answer. it's the nails

that are bothering me - i've never seen them all lift off in a very short

space of time. toes and toenails unaffected.
