Ductal yeast can be treated with *either* a loading dose of 400mg and then 200mg each of the other 13 days *or* a loading dose of 200 mg and then 100 mg the rest of the days. Oral fluconazole is overkill for a nipple infection. I don't think a mother needs to take oral med if it's skin-level, which it would be if it is just on the nipples. Why stress her liver with an oral med when a topical one will take care of the problem? A topical, like nystatin ointment applied in a thin coating 8x/24 hrs, after each feeding, should do the trick but must be continued for 10 days past the end of symptoms (usually a total of 14 days). If the mother only treats topically for 7 or 10 days, it seems to come back every time. Systemic candidasis is listed as 400mg loading with 200 daily for 28 days. No mentiopn of dosage for candida infection on nipples. Dee, do you think that the higher dosage is for ductal candida( systemic ) and possibly a lower dosage can be effective if just the nipple is infected?
