In my opinion there are many factors involved. Kotlow’s work on aerophagia, Mukai’s work on hyoid deviation, etc are all additions to the body of work on this topic. I also think that it is common to see other midline issues when tongue tie presents; even if those are issues we can't see with the naked eye. These babies will often hiccough more often, reflux more often, have more G.I. issues in general, etc. I think as well as the tongue tie, there can be some other midline issues such as immature esophageal sphincters, laryngomalacia, poor gut motility, hypospadias, etc. Sometimes these issues are quite apparent and other times they are just very borderline. However, due to the midline nature of tongue tie, it is not uncommon to see other concurrent midline “defects” in my experience. Tongue tie release, supportive positioning, anticipatory guidance for parents, structural/bodywork, gut healing, etc are all usually key in such cases.
