For me personally I find it highly paternalistic and in some cases negligent to *not* have a sensitive discussion about this condition when mother's present with it. Yes this falls along a spectrum, there are cases when it is not glaringly evident. And yes, sometimes all mother's hear is some LC telling her she wont make enough milk, which I suspect none of us have every said to any mother, nor would I. There is a compassionate way to discuss this however that doesn't mean that woman, in all her grief, is really going to hear how delicately it was approached.
