Hi Amy, n here from Austin...

I've had some problems at first after NHIC stopped processing these.

I had faxed a letter to TMHP (TX Medicaid & Healthcare Partnership) when

we began the evaluation process & was told that no prior authorization

for heart transplant eval is required & to submit medical necessity

documentation for transplant. The fax # to submit is 512 514-4213. Be

sure to include your provider # as well as the patients Medicaid #.

Their phone # is 800 925-9126.

Hello everyone,

What are the requirements for evaluation and transplant for someone with

Traditional Texas Medicaid? Is a letter of medical necessity required?

Are certain forms required? Where would I get this information? Pls

help, I am new to transplant and would appreciate any help or

information you can provide.

Thanks,
