Recurrent UTI

I would be greatful for any help with the following patient:

69 yr female C/O:

1. Recurrent cystitis for the last 5 years.

Symptoms: Not necessarily dysuria. Initially, general feeling of unwelness,

foul smelling urine

followed by lethargy and fatigue.

Infections confirmed by urine culture.

Frequent AB's in the past - at o/s of treatment, every 4 -6 weeks. Thrush

following AB's. Caniston cream/pessaries prn.

2. Acne rosacea (mild) diag. in '92. Typical facial distribution - cheeks

and bridge of nose. Also eyes affected - sticky and gritty with mucus

discharge, lids sore. GP Rx oxytetracycline with minimal effect.

3. Alopecia (mild), gradual hair thinning in last year.

PMH: Dilatation of urethra and bladder due to incomplete bladder emptying -

unsuccessful op.

Varicose vein op.

Appendectomy 19 yrs.

MS: OA in cervical spine, thumbs, fingers and Knees. Much pain, stiffness

and swelling.

NS: Much stress in past few years. Mild shingles out-break in December

GIT: BMvt: 0-1/d, no sig. constipation

Gyn: Menopause 45yrs

RS: Infrequent colds

Pulse: 69b/m reg, light BP: 130/80

Diet: Very good. Reg.execise.

DH: Currently taking Kwai garlic, Glucosamine

sulphate, flax seed caps 2000mg/d.

This patient has been taking herbal medicine on and off for the last 3 years

with variable effect on her health problems. (She tried homeopathy for two

years prior to that with minimal success). She has been given a wide range

of tinctures and herbal teas in different formulas - none of which seem to

acheive a sustained improvement. For example, the following Rx aggrivated

both the cystitis and OA.

Apium 1:1 10

Berberis aq. 15

Piper methisticum 20

Barosma betulina 30

Arctium lappa 10

Echinacea purp 30

Calendula 10 Sig. 7ml tds ac c aq

The patient responded better to the following Rx:

Apium 1:1 30

Echinacea purp 30

Menyanthes 15

Tarax rad 1:1 10

Juniperus 20 Sig. 5ml tds ac c aq

Plus a decotion of Equisetum with sugar

and an infusion with Zea mays, Barosma, Agropyron, and Althea

Eventually, after one year of herbal medicine, the UTI's

became less frequent and there was a general improvement in her skin and

joints. For the last two years, she had been AB free although had periods of

feeling unwell/fatigued most months and about 6 episodes of dysuria which

resolved

on their own.Then in December '99 she had a heavy fall, a bad cold followed

by an out break of shingles and two UTI's which required AB's. Since then

she feels she has been constantly " fighting off an infection. "

I have prescribed the following tonic tincture on which she feels brighter

but is still aware of a sub-clinical UTI:

Equisetum 20

Urtica fol 20

Astragalus 20

Eleutherococcus 40 Sig: 5ml tds

Overall, this patient tends to be better on the herbs than off, with more

energy, fewer UTI's, less joint pain and less florid Acne R. However,

nothing resolves completely and there have been continual flare-ups - when

the cystitis is good the OA is bad and vica versa. The patients main concern

is the recurrent UTI's and is disappointed that her immune

system does't seem to be able to cope when she is not taking herbs and she

is worried that with this recent state of lowered immunity, she will never

be with out medication - herbal or otherwise.

Things appear to have got stuck. Any suggestions ?

Hi Charlotte,

You might try elderberry juice,it can be of use for UTI's and help prevent

against other infections also.

Dear Charlotte,

An interesting case history...

One idea I tried on a Px with extremely chronic cystitis, and got very

lucky...

Px paraplegic so wheelchairbound due to spinal injury in RTA, was on

intravenous ABS after very long H. of oral ABS Tx. Urethral catheterisation

had to be terminated due to complications, and an abdominal wall ctheter

inserted. Doctors were despairing of what to do radically next. As patient

could feel nothing we had three points of reference:-

1) sense of well-being

2) smell of urine

3) (condition of) infection at catheter site.

A combination of Hydrastis 10-15g + Thymus 15-20g mixed and capsuled (size

00) dosage 2 tid and a tea of Arctostaphylos, Zea and Agropyron at 1oz in

one pint of boiling water etc. taken as three divided doses through the day

and night. Calendula cream with Tea tree oil for application to the abdomen.

Echinacea did feature mildly early on, but the capsules were the mainstay.

Tx. brought (over a few months) no smell in urine, generally healed abdo.

wall and an improvement in well-being.

In your Px. Hydrastis would help cool and clear the ? heat behind the

cea and deal with the catarrh etc. I would be very tempted to try a

nervine support for your Px. perhaps Verbena, this could help move any

emotions that may be stuck, and Bach flower remedies.
