The contract is in the article "A Tool for Safely Treating Chronic Pain". Click on Our Medication Use Agreement for a copy of the contract. Hope it helps. Patin Reno,NV Re: 'value added' fee Boundary setting is definately necessary at times, especially with borderline personalities...I found scheduling more frequent appointments with people who "need to talk" more helps. Usually weekly appointments for a month or so helps us determine what the real needs are, and then the phone isn't abused. If it continues, I have had to draw up a contract, like my narcotics contract, for two patients. I had to actually "fire" one based on the agreement of the contract. She enrolled in a 6 week Cognitive Behavioral Therapy program and then came back on better terms. I was, to note, actively involved in my own weekly psychoanalysis at the time to figure out why I let patients cross my boundaries...trying to save them all etc. It took two years and thousands of dollars...but it was worth it.
