I do house calls for hospice patients already (I'm the medical director of a small hospice in addition to my low overhead practice) but haven't done many for my regular patients. Can I query those in the goup who have an established solo practice with no or minimal staff - how long is your average visit? How many patients in a 3 hour time block are normal for you? I'm trying to judge how many patients a day will feel right when I'm not seeing 2-3 new patients every day. How long do people schedule for a physical? Guinn Yahoo! Guest guest Posted January 2, 2004 Report Share Posted January 2, 2004 Sure, I usually bill a 99345 or 99344 for new patients and a 99350 or 99349 for established patients. I accept the going medicare reimbursement. This will vary state to state. I also get a small fee (or at least they promised one) for being the medical director of a home health agency. I also get a bit of income on codes G0179, G0180, G0181 G0182 for home health patients of which I have many. When needed I perform an EKG, clean out impacted cerumen, PFTs, and Medicare pays a whopping $3.00 for me to perform a venipuncture. I also inject a few joints, Rocephin, etc. I also average 0-3 in-patients So I’m thinking that the average reimbursement per patient is about $120. 120x5x5x46=$138,000. Add on the home health recerts and hospital and you have a livable living with very appreciative patients, good satisfaction, unique position in the community. For example, adult protective services calls me to assist with rescuing abused elders. I give my cell phone number out to my patients, file papers into my electronic charts myself, pick up the mail myself, post payments myself. My wife answers the phone and comes with me on initial visits and does the billing. When needed, the hospitalists are glad to cover for me as I take their patients if they cannot arrange follow up in the home. I have someone who will cover for me when I am gone but this usually amounts to some phone calls and a rare Housecall only. I started doing this full time in SEP and I am no longer able to take new patients. The demand for this service is unbelievable. I am looking for a partner if anyone is interested. J.
