He sees all the pre transplant patients and then sees posts on a situational basis. We use a dietician for our evals (pre) only and if necessary for any follow-up. Guest guest Posted June 8, 2006 Report Share Posted June 8, 2006 Hi Angie, our dietician is a hospital employee budgeted to work w/us .25 FTE/wk and that works out to her being here during some part of post-op clinic 2-3 days/week. She sees all pre-tranplant pts as part of the evaluation process, and then sees post-op pts as needed. Our social worker is here .80 FTE, sees all pre-txp pts for education/orientation/psychosocial assessment, checks in w/them again at surgical eval time and then post-op is available prn until a final graduation exit interview. She updates the post-op care plan w/waitlisted pts by phone. I'm attaching their descriptions here that is given to our pre liver transplant patients. The Social Worker As a member of the transplant team, your child’s social worker focuses on the non-medical aspects of transplantation that can have an effect in every area of a person’s life. He or she works closely with your family and the team to help you understand and cope with your reaction to your child’s hospitalization and treatment, personal or family problems, financial concerns, and your return home. The social worker will, if needed, provide supportive counseling, education, and referrals to community resources as a way to work through the concerns that you and he or she identify. Transplant Dietician The transplant dietician is a nutritional expert who will work with you and your child during the entire transplant process. During the evaluation period, he or she will assess your child’s nutritional status and give you any necessary recommendations to optimize your child’s health prior to transplant. The dietician will work with you and the team during your child’s hospital stay to assure that your child receives adequate nutrition during this time. He or she will also discuss with you how your child’s nutritional needs will change after transplant. Finally, they will teach you how to change your child’s diet in order to meet those needs. She or he will continue to work with you after discharge to continually re-evaluate your child’s nutritional status. The dietician works closely with you to ensure that your child receives optimal nutritional care. They will see patient's both pre and post transplant as dictated by the transplant team and the patient's needs. -m 2a.
