I think it is an interesting approach to making primary care affordable by creating a personal financing stream outside of the current health care dollars. As there are already many patients having to shell out of pocket, this approach may help patients gain access. But this is only a piece of what we're after. In addition to creating affordable health care, we need to find the path to restructuring the flow of dollars. The amount of money in US health care is monumental, but the bulk goes to clean up the mess. We can prevent much of the mess if we have the right tools, the right team, and the time to do the work. As Larry pointed out, we have the relationship. The Care Model (www.improvingchroniccare.org) tells us how to craft the team and what tools we will need to do the work. The time it takes to deliver on the Care Model is not supported in the current financing paradigm. Most of the Pay for Performance (henceforth P4P) models are too small, too complex and too cheap. I have this insane vision that we can create a model of practice that encourages payers to truly fund the work it takes to achieve better population health.
