I too have been keeping a spread sheet of everyone including which EMR/PMS

they were using. I hope it's accurate.

To anyone not able to see attachments, I suspect your settings at yahoo

are for the daily digest. To receive attachments, you must change your

preferences to individual emails. Not as tidy, but the only way to get

all the attachments sent amongst the group members.

I'm not at home right now, but I'll try to re-send the xls spreadsheet

directly to your email address tomorrow.

rec of USPSTF offered or completed). Appointment slots are 30 and 60

minutes only. I have one staff, (my receptionist). I see between 6 and 12

patients per day, depending on the day. I do peds, GYN, No OB, Procedures

including minor surgery (lumps and bumps derm), colpo, flex sig, and some

others. I do inpatient and nursing home care. My partner runs a typical IM

solo practice with 3.5 staff and 20 plus patients per day. We take our own

call during the week and share weekends every other. (never terrible).

heavy into community education with hundreds of hours of lectures and

classes over the past 4 years that we have been open. (you can leave out

whatever is not needed or does not fit your flow sheet.) I am willing to be

contacted, and may contact others for further ideas and sharpening.

, how do you manage all the USPSTF A & B in every visit? Is this

built into templates and thus you just check off that you've covered it?

Hi :

You asked about how the USPSTF was managed every patient, every visit. Sure, I will be happy to share about that.

The only way this could happen is via a good medical records system with a rules engine. As I mentioned, the medical record I use is Logician by GE medical systems. You can see them on the AAFP web site. They have just signed an agreement with AAFP to work towards an "affordable, clinically efficient EMR that would eventually be available to all clinicians."

Yes, it does have templates and flow sheets. The templates and flow sheets help with input, but the engine does all the work. The parameters are set by the clinician and include such things as age, intervals, specific disease classifications, drugs, memos that are placed in a specific text field and other "observations" RE the USPSTF protocols that we have placed into the system. The data base searches for the data RE USPSTF on each patient. Those elements of the recommendations that are not fulfilled in the time frame set up will show on my screen. If all elements are satisfied the screen is blank.

If the patient has any elements that are not satisfied I get a reminder. As part of my vital signs I always cover meds, allergies, advanced directives and preventive care. It becomes a kind of a jingle or mantra that patients can eventually recite by wrote memory. The first time this data is entered into the system in it takes about 5 to 15 minutes depending on the individual. Usually each subsequent visit I only have to update that data and it takes 1 to 5 minutes of my 30 minute visit.

The USPSTF recommendations are "finite". Even though there seems to be many, many recommendations, there is an end to them. In my residency, (as Lane Lee will attest), we were taught a pneumonic GO WITCH, which was a reminder for those A and B recommendations. (The Pneumonic came to us thanks to Warner D.O. during our residency training at Florida Hospital). Guaiac (Colorectal Cancer Screening), Obesity, (BMI calculated each visit with BMI risk sheet covered as needed) Women's issues (Pap, BE, Mammo, HRT, Dexa, etc), Immunizations, Tobacco, Cholesterol, Hypertension. Once you cover the basics it is just a case of reminding the stragglers. I let them know they can tell me to stop asking any time, but then the decision to NOT provide care is theirs and is not on me. I have found I have less people than I can count on one hand that have requested this.

Eventually my screen clears and patients who never come for a wellness visit eventually consent to the care they often dread. It could not happen without my EMR. A simple act such as weighing every patient, every visit and calculating their BMI, plotting it on a graph and a risk curve for them (automatic and no extra time due to the EMR) never fails to draw comment and sometimes nets a decision. I have been fairly successful with such things as weight loss, dietary modification, and lifestyle change. This is simply due to consistency and the openings afforded by a good system that provides good data. Good information given repeatedly and placed on a trend helps people become convicted of their need for change. They become the ones that lead the discussion by actively asking what they need to change to be more healthy. (hmmm, well, funny you should ask....) Patients get to expecting this level of care and often they hop up on the scale or put their arm out for the b/p before I can ask. When this happens I see it as a small sign that they have bought into efforts to provide them with preventive care. Does not take much with a good EMR.

As I mentioned, the rules engine is "Key". I have reviewed several electronic medical records whose rules engines have been limited. The one I am on also has some limitations and I wish a few changes could be made. But, I have learned to manipulate the system I am working with and it serves me well at this time. So, lest you think I am some super-duper whirlwind of a doc, look again. Just the average guy with a little technology that I know how to use. Hope that answers your question.
