Guest guest Posted August 16, 2004 Report Share Posted August 16, 2004 http://www.lrc.state.ky.us/KRS/213-00/076.PDF 213.076 Certificate of death or provisional certificate of death required to be filed -- Medical certification of cause of death.snip/snip(3) The medical certification shall be completed, signed, and returned to the funeraldirector within five (5) working days after presentation to the physician, dentist, orchiropractor in charge of the patient's care for the illness or condition which resultedin death, except when inquiry is required by KRS 72.400 to 72.475. In such cases,the coroner shall complete and sign the certificate within five (5) days after receivingresults of the inquiry as required by KRS 72.400 to 72.475. In the absence of thephysician, dentist, or chiropractor, or with such person's approval, the certificatemay be completed and signed by his associate physician, dentist, or chiropractor, orthe chief medical officer of the institution in which death occurred, or the physicianwho performed an autopsy upon the decedent, or a physician employed by the localhealth department, if the individual has access to the medical history of the case anddeath is due to natural causes.
