Guest guest Posted December 16, 2003 Report Share Posted December 16, 2003 I practice part time - officially half-time in order to get a discount on my medical liability insurance, but actually flexibly and less than half-time because of slowly building a new practice to replace my former group practice job. I am on call 24/7 with my office phone forwarded to my cell phone, so I can make appointments any time my patient and I agree upon. I have this flexibility because I have no employees to pay or to schedule work hours for. I use one exam room in the conventional office of another family physician, and my phone and multifunction printer vanish into a closet when I'm not there. In fact, they are mounted on a closet shelf, so I just open a small door of a built-in cabinet and plug in my computer, and I'm on line. I installed an encrypted wireless router to get free broadband DSL Internet access from my landlord physician. My notebook computer with EMR goes home with me, so I practice out of a briefcase. I don't have family responsibilities (apart from " having a life " with my wife), but I still have a part-time medical management position which also needs flexibility. (Although building a new practice is slow in this over-supplied community, I am not " quitting my day job " to do it. I am not in financial or emotional distress, and borrowed no money to start this practice. I have no debts.) Medrium costs 75 cents per bill for online electronic billing, with a minimum of $25/month. You can print out your own balance-billing statements to patients periodically, or give that also to Medrium for 60 cents per statement including printing, folding, stuffing, postage and mailing. Basic financial reports are included, or you can buy an additional package for some fancier stuff that I do not need. That's my Practice Management System, and it charges my credit card every month just like my phone bills. No up-front costs. The only downside I have with Medrium is that it does not currently have a "seamless" interface with my EMR (Praxis, the version 3 in beta testing stage), but that is expected to end soon with an "interface engine" from Praxis. It means that the billing person (my lovely wife) has to enter in demographics and whatnot but to be perfectly honest, I like having this one piece of paper as another layer of backup in case…Once the demographics are entered, the rest is really easy. To ask a personal question, can you make a living seeing five patients a day? There was one housecall doctor here in Albuquerque who moved away last year. His patients really liked him - they did tell me that it was rare for insurance to reimburse him, as they weren't absolutely "housebound" by Medicare standards. It’s just me and my wife who does billing and phones. I’ve only been doing this for three moh=nths and my practice is absolutely full. As I metabolize this growth, I plan to incorporate a NP or two but ideally would love to have another physician partner. Look up medicare codes 99345 and 99350—reimbursement only requires that the patient has a good reason why a Housecall is needed-it is not like home health. I do house calls for hospice patients already (I'm the medical director of a small hospice in addition to my low overhead practice) but haven't done many for my regular patients. Can I query those in the goup who have an established solo practice with no or minimal staff - how long is your average visit? How many patients in a 3 hour time block are normal for you? I'm trying to judge how many patients a day will feel right when I'm not seeing 2-3 new patients every day. How long do people schedule for a physical? Established pts, for a URI (if they want to be seen) 15 mins, physical 60 mins, FU of chronic dz 30-60 mins. New pt physical 60-90 mins.
