Insurance question

Hi All,

I have a patient being scheduled for living unrelated kidney surgery

next week. He is insured through his stepfather who he lives with, but

now we find that his natural father is also carrying insurance, Cigna.

He also has Medicare, which is third.

The question is who's insurance would be prime, the step father's or

the natural father. Would the birthday rule apply?

I plan to auth with both insurances, but there is a chance that the

natural father's plan, Cigna, may be out of network.

Appreciate any advice.
